About Mounjaro

Mounjaro, manufactured by Eli Lilly, contains tirzepatide as the active ingredient. It is a once weekly injection, recently approved in the UK for weight loss and weight management. Mounjaro is approved for use in adults with a BMI of 30kg/m² or more (obesity), as well as those with a BMI of 27 to 30kg/m², who also have other weight-related health problems. Mounjaro is used in combination with increased physical activity and a reduced-calorie diet.

Tirzepatide works on glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) receptors, increasing the production of insulin in the body, and helping to reduce blood sugar levels. Mounjaro also slows down emptying of the stomach after a meal creating a feeling of fullness, making you feel less hungry.

What are the most common side effects of Mounjaro?

Side effects of Mounjaro are split into common and serious.

In clinical trials common side effects included:

  • Nausea, vomiting
  • Diarrhoea, constipation
  • Low blood sugar (when used with certain diabetes medications)
  • Loss of appetite, indigestion, abdominal pain
  • Dizziness, low blood pressure, tiredness
  • Allergic reactions

Serious side effects include:

  • Pancreatitis (inflamed pancreas)
  • Gallstones
  • Gastroparesis
  • Hypoglycemia (very low sugar levels)

There is not enough evidence to suggest there are any drug interactions between herbal supplements and Mounjaro. In any event, you should let your prescribing clinician know if you are taking any herbal supplements or other prescription drugs.

Why might Mounjaro not work for weight loss?

Certain health factors may mean you do not lose weight when using Mounjaro. Also, as with other medications, there are a number of people, classed as non-responders, for whom Mounjaro will have no effect.

Can Mounjaro taken for weight loss be taken long term?

In clinical trials, Mounjaro was used for 72 weeks (18 months), when used to help reduce body weight. In the U.S., Mounjaro is FDA-approved for long-term use in patients with diabetes. As with all medications however, Mounjaro comes with side effects and has increased risks for certain medical conditions.

Is there a herbal Mounjaro?

No. Herbal Mounjaro does not exist. If you have been marketed such a formulation, it may be fake.

Can women use Mounjaro?

Women can use Mounjaro for the treatment of diabetes or weight loss. It is not recommended however during pregnancy or breastfeeding.

Should I avoid any particular food or drink when taking Mounjaro?

What is Mounjaro?

Mounjaro is a prescription medication containing tirzepatide, a dual GIP and GLP-1 receptor agonist administered as a once-weekly subcutaneous injection. It is manufactured by Eli Lilly. Mounjaro is licensed in the UK for the treatment of type 2 diabetes mellitus and has also been approved for chronic weight management. It is the first dual incretin agonist, targeting both glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors, providing superior glycaemic control and weight loss compared to single-target GLP-1 agonists.

How does Mounjaro work?

Mounjaro works by simultaneously activating two incretin hormone receptors: GIP and GLP-1. This dual mechanism enhances insulin secretion in a glucose-dependent manner, suppresses glucagon release, slows gastric emptying, and reduces appetite through central nervous system pathways. The combined GIP/GLP-1 action provides greater efficacy than GLP-1-only medications. In clinical trials (SURMOUNT programme), tirzepatide produced average weight loss of 15–22% of body weight — significantly greater than semaglutide (Wegovy), making it the most effective weight loss medication currently available.

Is Mounjaro available on the NHS?

Mounjaro has been approved by NICE for type 2 diabetes management on the NHS. For weight management, NICE is evaluating tirzepatide and NHS access may depend on local commissioning arrangements and specialist weight management service capacity. Mounjaro is also available privately through online prescribing services like NowPatient for eligible patients. Your prescriber will assess your suitability based on BMI, comorbidities, and medical history.

How do I inject Mounjaro?

Mounjaro is injected subcutaneously once weekly using a pre-filled pen (KwikPen). Inject into the abdomen, thigh, or upper arm, rotating injection sites each week. Inject on the same day each week at any time, with or without food. The pen automatically inserts the needle when you press the injection button — hold for 10 seconds until the grey plunger is visible and you hear two clicks. Each pen is single-use. Safely dispose of used pens in a sharps bin.

What is the dose escalation for Mounjaro?

Mounjaro follows a gradual dose escalation: start at 2.5 mg once weekly for 4 weeks, then increase to 5 mg once weekly. Further increases can be made in 2.5 mg increments every 4 weeks as tolerated, up to a maximum of 15 mg once weekly. The available doses are 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg. Your prescriber will determine the optimal maintenance dose based on your response and tolerability. Do not increase doses faster than every 4 weeks.

What should I do if I miss a dose of Mounjaro?

If you miss a dose and the next scheduled dose is more than 4 days (96 hours) away, take the missed dose as soon as possible. If fewer than 4 days remain before the next dose, skip the missed dose and resume the regular schedule. If more than 4 weeks pass without a dose, contact your prescriber as you may need to restart at a lower dose. Never take two doses within 3 days of each other.

What are the common side effects of Mounjaro?

Very common side effects (affecting more than 1 in 10 people) include nausea, diarrhoea, vomiting, constipation, and decreased appetite. These gastrointestinal side effects are most pronounced during dose escalation and typically improve over time. Common side effects include abdominal pain, dyspepsia, injection site reactions, fatigue, hypersensitivity reactions, eructation (belching), flatulence, gastro-oesophageal reflux, and hair loss. Eating smaller meals, avoiding fatty foods, and adhering to the gradual dose escalation minimise GI symptoms.

Can Mounjaro cause pancreatitis?

As with other incretin-based therapies, there is a potential risk of acute pancreatitis with Mounjaro. Symptoms include severe, persistent abdominal pain often radiating to the back, with nausea and vomiting. If you experience these symptoms, stop Mounjaro and seek immediate medical attention. Patients with a history of pancreatitis should discuss risks with their prescriber. The overall risk appears low in clinical trials, but vigilance is advised.

How is Mounjaro different from Wegovy?

Both are injectable weight management medications but work through different mechanisms. Mounjaro (tirzepatide) targets both GIP and GLP-1 receptors, while Wegovy (semaglutide) targets only GLP-1 receptors. Head-to-head trials show Mounjaro produces greater weight loss (15–22% vs 12–15%) and greater HbA1c reduction for diabetes. Both are administered weekly. The choice depends on availability, cost, individual response, and prescriber recommendation. Mounjaro is currently the most effective injectable weight loss medication available.

Who should not take Mounjaro?

Mounjaro is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2). It should not be used during pregnancy or breastfeeding. Caution is advised in patients with a history of pancreatitis, severe gastrointestinal disease, diabetic retinopathy, and severe renal impairment. Mounjaro should not be combined with other GLP-1 receptor agonists.

How should I store Mounjaro?

Before first use, store Mounjaro pens in the refrigerator at 2–8°C. Do not freeze. Mounjaro can be kept at room temperature (up to 30°C) for up to 21 days if needed. Protect from direct heat and light. Do not use if the solution is cloudy, discoloured, or contains particles. Each pen is for single use only.
