Telehealth Consent

Version: 1.0
Last Updated: 7 Oct 2024

The purpose of this consent form (“Consent”) is to provide you with information about telehealth and to obtain your informed consent to the use of telehealth in the delivery of healthcare services to you by pharmacists and clinicians (each a “Provider”) using our telehealth practice platforms – the Website (NowPatient) or App (NowPatient) – owned and operated by Infohealth Limited (“Infohealth, “we” or “us”) (the “Service”)

Your Consent is deemed to be accepted by you using the Service.

Terms

Other terms also apply to your use of the Website and the App to use the Services. These are set out on our Website and include (but are not limited to) the following:

If you don’t grant your Consent, you should not use the Service.

Benefits to using Telehealth

“Telehealth” is the use of electronic media to interact with a Provider.

  • Telehealth is a well-accepted method of delivering health care that is easier for the patient to use and can usually be scheduled at the patient’s convenience.
  • Unless recommended by your Provider, you won’t have to visit a clinic or hospital to see a healthcare professional. This reduces your risk of getting sick from other people.

Risks to using Telehealth

For many types of Provider interactions, a telephonic or Video Consultation (as defined in our Terms of Use) is sufficient to make a diagnosis and treat. It is important that you understand and Consent to the limitations inherent in receiving a diagnosis without a physical examination.

  • You and your Provider won’t be in the same room, so it may feel different than an office visit.
  • Your Provider may make a mistake because they cannot examine you as closely as at an office visit.
  • Your Provider may decide that you still need an office visit.
  • Technical difficulties may occur before or during the telehealth sessions and your appointment cannot be started or ended as intended. Your Provider may commence a Video Consultation; however you understand that the session may need to be conducted by regular voice communication if technical requirements such as internet speed cannot be met.
  • Your current insurance may not cover the additional fees of the Service and you may be responsible for any fee that your insurance company does not cover.
  • There is a risk that your data or information could be lost or stolen. We will do our best to limit that possibility and to notify you if we identify any breach (in accordance with our Privacy Policy).
  • No results are guaranteed or promised by using the Service.

You understand these risks and benefits, and Consent to using the Service. You have the right to withdraw your Consent to the use of the Service at any time. This will not affect your right to future care or treatment.

You understand that your Provider is compelled by applicable law and professional standards to report certain events, particularly if they believe that you or someone else is in danger.

Emergency Care

If your situation is an emergency, call 999 from the UK or 911 from the US. Telehealth is not an appropriate method of care. If you require immediate or urgent care, you must seek care at a hospital or other healthcare provider equipped to deliver urgent or emergent care.
