About Elleste Duet
Elleste Duet contains two active ingredients, estradiol, a natural version of oestrogen and norethisterone, a synthetic progestogen. It is classed as a HRT (hormone replacement therapy), used to treat the symptoms of menopause. Symptoms of menopause include:
- Osteoporosis
- Hot flushes, night sweats
- Depression, mood swings
- Sleep problems
- Vaginal dryness
Elleste Duet is a combined treatment used in women who are suffering from menopause, but still have a menstrual cycle, and have not had a hysterectomy.
If only oestrogen levels are increased during HRT treatment, the endometrium would grow excessively, causing an increased risk of endometrial cancer. Women who have had a hysterectomy are prescribed Elleste Solo tablets.
NowPatient offers Elleste Duet as a treatment for HRT only, following an online consultation with a prescribing clinician.

FAQs
Answers to Popular Questions About Elleste Duet
What is Elleste Duet?
Elleste Duet is a combined sequential HRT product containing oestradiol and norethisterone acetate. It is available as Elleste Duet 1 mg (1 mg oestradiol + 1 mg norethisterone) and Elleste Duet 2 mg (2 mg oestradiol + 1 mg norethisterone). The pack contains 16 white oestrogen-only tablets followed by 12 green combined (oestrogen + progestogen) tablets. It is used for menopausal symptom relief in women with an intact uterus who are perimenopausal or within 12 months of their last period.
How does Elleste Duet work?
Elleste Duet provides oestradiol continuously to relieve menopausal symptoms, with norethisterone added for 12 days each cycle to protect the womb lining (endometrium) from the thickening effect of unopposed oestrogen. This sequential pattern produces a predictable monthly withdrawal bleed, similar to a period, during or after the progestogen phase. This is the standard approach for women who are still having some periods (perimenopausal) or have been menopausal for less than 12 months.
What is the difference between Elleste Duet and Elleste Solo?
Elleste Solo contains oestradiol only and is intended for women who have had a hysterectomy. Elleste Duet combines oestradiol with norethisterone (a progestogen) and is intended for women with an intact uterus, as the progestogen protects the womb lining. If you still have your womb, you need a combined HRT product or must add a separate progestogen to oestrogen-only HRT.
How do I take Elleste Duet?
Take one tablet daily at the same time. Start with the 16 white (oestrogen-only) tablets, then take the 12 green (oestrogen + progestogen) tablets. After finishing the 28-tablet pack, start a new pack immediately without a break. A withdrawal bleed usually occurs during or just after the green tablet phase. Take the tablets in the correct order as marked on the pack.
When should I switch to continuous combined HRT?
Sequential HRT (like Elleste Duet) produces a monthly withdrawal bleed. Once you have been postmenopausal for at least 12 months, your prescriber may recommend switching to a continuous combined HRT (such as Kliofem or Kliovance), which aims to be bleed-free. Switching too early may cause irregular breakthrough bleeding. Your prescriber will advise on the optimal timing.
What are the side effects of Elleste Duet?
Common side effects include breast tenderness, headaches, nausea, bloating, and mood changes. The progestogen phase (green tablets) may cause PMS-like symptoms including mood changes, bloating, and breast tenderness. Breakthrough bleeding outside the expected withdrawal bleed may occur in the first 3 months. Leg cramps and skin reactions are uncommon.
What are the risks of Elleste Duet?
Combined HRT (oestrogen + progestogen) is associated with a small increased risk of breast cancer with use beyond 5 years. The risk increases with duration and returns to baseline within a few years of stopping. Oral HRT carries a small increased VTE risk. Transdermal oestrogen with micronised progesterone (Utrogestan) may offer a lower-risk profile. Your prescriber will discuss your individual risk-benefit balance.
Is Elleste Duet body-identical HRT?
Elleste Duet contains body-identical oestradiol (17β-estradiol, identical to the oestrogen produced by the ovary), but the progestogen component (norethisterone acetate) is a synthetic progestogen, not body-identical progesterone. For a fully body-identical regimen, some prescribers recommend oestradiol (tablets, patches, or gel) combined with micronised progesterone (Utrogestan). Discuss your preferences with your prescriber.
How can I get Elleste Duet?
Elleste Duet is available on NHS prescription from your GP, menopause clinic, or through online services like NowPatient. Your prescriber will determine whether sequential or continuous HRT is appropriate based on your menopausal stage. NowPatient offers online consultations and home delivery.
