Mysimba Key Facts
About Mysimba
Mysimba contains a combination of bupropion hydrochloride, an antidepressant, and naltrexone hydrochloride, an opioid antagonist as its active ingredients, both of which help with weight loss.
Mysimba is an anti-obesity medication, used with a reduced-calorie diet and increased physical activity, for weight management in adults with a body mass index (BMI) of 30 kg/m2, or 27 kg/m2 to < 30 kg/m2, with one or more weight-related health conditions, such as high blood pressure, high cholesterol, or type 2 diabetes. It is thought the naltrexone/bupropion combination reduces a person’s appetite and increases the amount of energy used up by the body.
NowPatient offers Mysimba as a treatment for weight loss only, following an online consultation with a prescribing clinician.

FAQs
Answers to Popular Questions About Mysimba
What is Mysimba?
Mysimba is a prescription oral weight management medication containing two active ingredients: naltrexone hydrochloride 8 mg and bupropion hydrochloride 90 mg per prolonged-release tablet. It is licensed in the UK for chronic weight management in adults with a BMI of 30 or above, or 27 or above with weight-related comorbidities, as an adjunct to a reduced-calorie diet and increased physical activity. Mysimba is an oral alternative to injectable GLP-1 weight loss medications.
How does Mysimba work?
Mysimba works through two complementary mechanisms. Naltrexone is an opioid antagonist that blocks the brain’s opioid reward pathways, reducing food cravings and the pleasurable associations with eating. Bupropion is a dopamine and noradrenaline reuptake inhibitor that acts on the hypothalamic appetite centre to reduce appetite and increase energy expenditure. Together, these two drugs target the mesolimbic reward system and hypothalamic regulation of appetite, addressing both the craving and hunger aspects of overeating.
How much weight can I lose with Mysimba?
In clinical trials (COR programme), patients taking Mysimba alongside lifestyle changes lost an average of 5–8% of their body weight over 56 weeks. Approximately 50% of patients achieved at least 5% weight loss. Weight loss with Mysimba is generally more modest than with GLP-1 receptor agonists like Wegovy (12–15%) but significant enough to improve weight-related health markers. Response should be assessed after 16 weeks — if less than 5% weight loss is achieved, treatment should be reconsidered.
What is the dosing schedule for Mysimba?
Mysimba uses a 4-week dose escalation: Week 1: 1 tablet in the morning; Week 2: 1 tablet morning and 1 tablet evening; Week 3: 2 tablets morning and 1 tablet evening; Week 4 onwards: 2 tablets morning and 2 tablets evening (maintenance dose of 4 tablets daily). Tablets should be swallowed whole with water, preferably with food. Do not crush, break, or chew the prolonged-release tablets. Take morning and evening doses approximately 12 hours apart.
What should I do if I miss a dose?
If you miss a dose, skip the missed dose and take the next dose at your usual time. Do not take a double dose. Continue with the regular dosing schedule. Consistency is important for maintaining therapeutic levels and getting the best results from Mysimba.
What are the common side effects of Mysimba?
Very common side effects include nausea (most common, affecting about 1 in 3 people), constipation, vomiting, and headache. These are most pronounced during the dose escalation phase. Common side effects include dizziness, dry mouth, insomnia, anxiety, tremor, abdominal pain, and increased heart rate. Nausea typically improves after the first 2–4 weeks. Taking Mysimba with food and following the gradual dose escalation helps minimise side effects.
Does Mysimba affect mental health?
Bupropion (one of the components) has been associated with psychiatric side effects including anxiety, insomnia, irritability, and rarely suicidal thoughts. Patients with a history of depression, anxiety, or suicidal ideation should be monitored closely when starting Mysimba. If you experience significant mood changes, agitation, or thoughts of self-harm, stop Mysimba and contact your prescriber immediately. Mysimba is contraindicated in patients with current or history of bipolar disorder, eating disorders (bulimia or anorexia), or seizure disorders.
Who should not take Mysimba?
Mysimba is contraindicated in people with uncontrolled hypertension, seizure disorders or history of seizures, current or history of bulimia or anorexia nervosa, bipolar disorder, current use of opioid-containing medications or during opioid withdrawal, severe hepatic impairment, and end-stage renal failure. It should not be used with MAO inhibitors. Caution is needed with drugs that lower the seizure threshold. Mysimba should not be used during pregnancy or breastfeeding.
Can I drink alcohol while taking Mysimba?
Alcohol consumption should be minimised or avoided while taking Mysimba. Bupropion lowers the seizure threshold, and alcohol further increases seizure risk. Additionally, abruptly stopping heavy alcohol use while on Mysimba increases seizure risk. Naltrexone blocks opioid receptors, which may alter the subjective experience of alcohol. If you regularly drink significant amounts of alcohol, discuss this with your prescriber before starting Mysimba.
How can I get Mysimba?
Mysimba is available on private prescription through your GP or online services like NowPatient. NHS availability varies by area and commissioning policy. NowPatient offers online consultations with qualified UK prescribers who can assess your eligibility and prescribe Mysimba with home delivery.
