About Estradiol pessaries

Estradiol pessaries are classed as hormone replacement therapy (HRT). They act in the same way as the female sex hormone oestrogen. They are used to treat vaginal atrophy and for the prophylaxis of recurring urinary tract infections (UTIs) in postmenopausal women. Estradiol pessaries help to treat menopausal symptoms such as vaginal dryness and irritation.

What are the side effects of Estradiol pessaries?

Side effects of Estradiol pessaries are split into common and serious.

Common side effects include:

  • Your skin, tongue or lips turn blue, grey or pale (if you have black or brown skin, this may be easier to see on the palms of your hands or soles of your feet)
  • You suddenly become very confused, drowsy or dizzy
  • Someone faints and cannot be woken up

There is not enough evidence to suggest there are any drug interactions between herbal supplements and Estradiol pessaries. In any event, you should let your prescribing clinician know if you are taking any herbal supplements or other prescription drugs.

Can Estradiol pessaries used for HRT be used long term?

HRT should only be used as a short-term treatment for the menopause. Women who use HRT for more than 5 years may be at an increased risk of ovarian cancer. Speak to your GP to review your treatment at least once each year and have regular checkups.

Is there a herbal Estradiol pessary?

No. Herbal Estradiol pessaries do not exist. If you have been marketed such a formulation, it may be fake.

Should I avoid any particular food or drink when using Estradiol pessaries?

You can eat and drink normally when using Estradiol pessaries.

What is Estradiol?

Estradiol (oestradiol, 17β-estradiol) is a body-identical form of the main female sex hormone oestrogen. In HRT, it is used to replace the oestrogen that the ovaries stop producing during and after menopause. Estradiol is available in multiple formulations: tablets (e.g., Elleste Solo, Zumenon), transdermal patches (e.g., Evorel, Estradot), transdermal gel (e.g., Oestrogel, Sandrena), and vaginal preparations (e.g., Vagifem). Body-identical estradiol is recommended by NICE and BMS as the preferred oestrogen for HRT.

Why is body-identical Estradiol preferred?

Body-identical estradiol (17β-estradiol) is molecularly identical to the oestrogen produced by the human ovary. This distinguishes it from conjugated equine oestrogens (used in Premarin). Body-identical oestradiol has a well-established safety profile, and transdermal forms do not increase VTE risk. NICE, BMS, and FSRH guidelines recommend body-identical hormones as the first-choice HRT option.

What Estradiol formulations are available?

Tablets: Elleste Solo, Zumenon, Progynova (1 mg and 2 mg). Patches: Evorel, Estradot, FemSeven (various strengths). Gels: Oestrogel (pump-dispenser), Sandrena (sachets). Which route of Estradiol is safest?

Transdermal estradiol (patches or gel) is considered the safest route because it does not undergo first-pass liver metabolism and does not increase VTE or stroke risk. It is particularly recommended for women over 60, those with obesity, migraine, VTE risk factors, or liver disease. Oral estradiol is also safe for most women but carries a small VTE risk.

What are the side effects of Estradiol HRT?

Common side effects include breast tenderness, headaches, nausea (oral), bloating, and mood changes. These usually settle within 3 months. Transdermal formulations may cause skin irritation at the application site. Side effects are generally dose-related — your prescriber will use the lowest effective dose.

Do I need progestogen with Estradiol?

If you have an intact uterus, you must take a progestogen (such as Utrogestan, norethisterone, or dydrogesterone) alongside systemic estradiol to protect the endometrium. If you have had a hysterectomy, estradiol alone is sufficient. Vaginal estradiol at low doses does not require progestogen.
