How Report Hospital Admissions Works

What Is the Hospital Admissions Reporting Service?

Hospital Admissions Reporting is a proactive health monitoring service that helps you track your hospital visits and stays while contributing to wider medical research. Every three months, NowPatient will prompt you to report any hospital admissions you may have experienced. This regular check-in creates a comprehensive record of your healthcare utilisation, helping you understand patterns in your health and communicate more effectively with your healthcare providers.

The reporting process is designed to be quick and straightforward. You will be asked about when you were admitted, the reason for your admission, how long you stayed, and the outcome of your visit. This information is securely stored in your personal health record and can be reviewed at any time. Your contributions support healthcare planning, medical research, and health system improvements, while also providing you with personalised insights into your health journey over time.

You are also able to make ad-hoc reports, if needed.

How to Report Unplanned Hospital Admissions

NowPatient allows you to not only submit ad-hoc hospital admission reports but also sends out a reminder every three months to make a submission, if applicable.

Interval Reminder for Unplanned Hospital Admission

NowPatient will send you a reminder notification every three months (quarterly period) to prompt you to report any hospital admissions you may have had.

Just follow the instructions on the notification and click the button that applies to you: ‘Report Hospital Admission’ or ‘No Quarterly Hospital Admissions to Report’

If you click ‘Report Hospital Admission’, you will be taken to the report page to complete your submission.

Reporting Ad-hoc or Unplanned Hospital Admissions

Navigate to the ‘Report Hospital Admissions’ Service Card which can be found in the Health Surveillance section of your account homepage.

From the Options menu, select, ‘Report Hospital Admission’.

You will now be prompted with a series of questions, which will handle the unplanned hospital admission report. Once completed, you do not need to take any further action.

Viewing Unplanned Hospital Admissions & Producing Reports to Share With Your Doctor

Navigate to the ‘Report Hospital Admissions’ Service Card which can be found in the Health Surveillance section of your account homepage.

From the Options menu, select, ‘View Hospital Admissions’.

Here you can view all the historical unplanned hospital admission reports you have made. You can open them individually to view.

You can also generate and download a report to your documents folder. To download the reports, use the data range selector, and then choose the reports you wish to export. Once you have selected the reports, click the ‘Export’ and this will send the aggregate report to your documents folder, from where you can share with your Clinician.

What Should I Report?

Report any hospital admission, including:

  • Emergency department visits that led to admission
  • Planned or scheduled hospital admissions
  • Surgical procedures requiring hospital stay
  • Observation stays
  • Intensive care unit admissions
  • Transfers between hospitals

For each admission, you will be asked to provide:

  • Date of admission and discharge
  • Reason for admission (diagnosis or symptoms)
  • Name of hospital or healthcare facility
  • Length of stay
  • Any procedures or surgeries performed
  • Outcome (fully recovered, ongoing treatment, etc.

What Happens If I Log an Unplanned Hospital Admission?

When you log an unplanned hospital visit, NowPatient will check whether you have completed a Patient Engagement Score assessment. This assessment measures how actively you are managing your health. Your score helps NowPatient personalise its support to match your understanding of your condition. With the right tools and tailored guidance, NowPatient can help you avoid preventable hospital visits in the future.

Is My Data Private and Secure?

Yes. Your hospital admission reports are stored securely in your personal NowPatient account and are protected by healthcare-grade encryption in line with Information Governance and HIPAA protocols. Your personal information is never shared without your explicit consent.

Is the Hospital Admissions Reporting Service Free?

Yes. The Hospital Admissions Reporting service is free for all registered NowPatient users.
