Ranexa is an antianginal that is used to treat a certain type of long-term chest pain (chronic angina).

How does Ranexa work?

Ranexa is an antianginal medication. Angina (chest pain) happens when the heart muscle isn’t getting enough oxygen. The way Ranexa works to treat angina isn’t completely understood. But it’s thought to help your heart work more efficiently by changing the flow of electrolytes in the heart. This might help lower the symptoms of angina.

How do I take Ranexa?

Ranexa is typically started at 500 mg by mouth twice a day. Your provider may raise the dose to a maximum of 1,000 mg twice a day, based on how you respond to the medication.

Is it safe for me to take Ranexa?

Ranexa is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication if you have liver cirrhosis or are taking a strong CYP3A inhibitor (e.g., ketoconazole, clarithromycin, lovastatin) or a CYP3A inducer (e.g., rifampin, phenobarbital, St. John’s wort).

Common side effects of Ranexa:

Ranexa Serious Side Effects

Serious side effects are rare with Ranexa. Contact your healthcare provider immediately if you experience any of the following.

  • Abnormal heart rhythm: abnormal heart rate, dizziness, difficulty breathing, chest pain, fainting
  • Serious kidney problems: tiredness, blood or foam in your urine, urinating more often, muscle cramping, swollen feet or ankles, poor appetite

Effects of other drugs, drug classes and over-the-counter products on Ranexa

The following medications may interact with Ranexa:

  • CYP3A inhibitors: Do not use Ranexa with strong CYP3A inhibitors. With moderate 3A inhibitors (e.g., diltiazem
  • verapamil, erythromycin), limit maximum dose of Ranexa to 500 mg twice daily.
  • CYP3A inducers: Do not use Ranexa with CYP3A inducers
  • P-gp inhibitors (e.g., cyclosporine): May need to lower Ranexa dose based on clinical response
  • Drugs transported by P-gp or metabolized by CYP2D6 (e.g., digoxin, tricyclic antidepressants): May need reduced doses of these drugs when used with Ranexa.
