Olmesartan/Amlodipine is an ARB / CCB / Diuretic (water pill) and is used to treat high blood pressure.

How does Olmesartan/Amlodipine work?

Olmesartan/Amlodipine is a combination of three medications that lower blood pressure. Olmesartan is an angiotensin II receptor blocker (ARB). Angiotensin II is a hormone that normally attaches to its signaling proteins (receptors) in the body to raise your blood pressure. Olmesartan blocks this interaction from happening to lower your blood pressure. Amlodipine is a calcium channel blocker (CCB). It works by relaxing your blood vessels to lower your blood pressure. Hydrochlorothiazide is a thiazide diuretic (water pill). It works in your kidneys to remove extra water and certain electrolytes, like sodium and potassium. This helps lower blood pressure.

How do I take Olmesartan/Amlodipine?

The typical starting dose for Olmesartan/Amlodipine is 20 mg/5 mg/12.5 mg (Olmesartan/Amlodipine/hydrochlorothiazide) by mouth once daily. Your provider will adjust your dose based on how well it’s helping your blood pressure and if you’re having side effects. The maximum dose is 40 mg/10 mg/25 mg (Olmesartan/Amlodipine/hydrochlorothiazide) once daily.

  • Dangerously low blood pressure: dizziness, lightheadedness, pale skin, feeling faint or fainting
  • Heart attack: tightness and pain in the chest, neck, arms, or back; cold and clammy skin; sweating; dizziness; tiredness
  • Kidney problems: urinating less often than usual; swelling in your feet, ankles, or hands; unexplained weight gain
  • High blood potassium levels: nausea, muscle weakness, abnormal heartbeat
  • Severe diarrhea

Effects of other drugs, drug classes and over-the-counter products on Olmesartan/Amlodipine

The following medications may interact with Olmesartan/Amlodipine:

Olmesartan medoxomil:

  • Nonsteroidal anti-inflammatory drugs (NSAIDS): May lead to increased risk of renal impairment and loss of antihypertensive effect.
  • Dual inhibition of the renin-angiotensin system: Increased risk of renal impairment, hypotension, and hyperkalemia.

Amlodipine:

  • If simvastatin is co-administered with amlodipine, do not exceed doses
  • greater than 20 mg daily of simvastatin.

Hydrochlorothiazide:

  • Alcohol, barbiturates, narcotics: Potentiation of orthostatic hypotension
  • Antidiabetic drugs: Dosage adjustment of antidiabetic may be required
  • Cholestyramine and colestipol: Reduced absorption of thiazides
  • Corticosteroids, ACTH: Electrolyte depletion, hypokalemia
  • Lithium: Reduced renal clearance and high risk of lithium toxicity when used with diuretics. Should not be given with diuretics
  • NSAIDs: Can reduce the diuretic, natriuretic, and antihypertensive effects of diuretics
