What is alteplase used for?
alteplase is a tissue plasminogen activator (tPA) indicated for the restoration of function to central venous access devices as assessed by the ability to withdraw blood.
How does alteplase work?
alteplase binds to the fibrin-plasminogen complex in a blood clot and converts the inactive plasminogen into active plasmin which digests the fibrin. ACTIVASE can dissolve a clot in either the coronary or pulmonary artery, restoring the blood supply to the area beyond the blockage.
How do I take alteplase?
alteplase is typically dosed as follows:
- Patients weighing ≥30 kg: 2 mg in 2 mL
- Patients weighing < 30 kg: 110% of the internal lumen volume of the catheter, not to exceed 2 mg in 2 mL
- If catheter function is not restored at 120 minutes after 1 dose of alteplase, a second dose may be instilled
Is it safe for me to take alteplase?
alteplase is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication if you have any of the following.
- Active internal bleeding
- Recent intracranial or intraspinal surgery or serious head trauma
- Intracranial conditions that may increase the risk of bleeding
- Bleeding diathesis
- Current severe uncontrolled hypertension
- Current intracranial hemorrhage
- History of recent stroke

- Anticoagulants and drugs that inhibit platelet function increase the risk of bleeding when administered with ACTIVASE therapy
- Concomitant angiotensin-converting enzyme inhibitors may increase the risk of orolingual angioedema
