What is Activase?

Activase is the brand name for alteplase IV injection.

What dosages is Activase available in?

Activase is available in strengths of 50 mg and 100 mg.

What is Activase used for?

Activase is a tissue plasminogen activator (tPA) used for the treatment of:

  • Acute Ischemic Stroke (AIS)
  • Heart attack
  • Acute Massive Pulmonary Embolism (PE) for lysis

How does Activase work?

Activase binds to the fibrin-plasminogen complex in a blood clot and converts the inactive plasminogen into active plasmin which digests the fibrin. Activase can dissolve a clot in either the coronary or pulmonary artery, restoring the blood supply to the area beyond the blockage.

How do I take Activase?

Activase is typically dosed as follows:

  • Acute Ischemic Stroke: The recommended dose is 0.9 mg/kg (not to exceed 90 mg total dose) infused intravenously over 60 minutes with 10% of the total dose administered as an initial bolus over 1 minute
  • Acute Myocardial Infarction: The recommended total dose is based on patient weight, not to exceed 100 mg
  • Acute Massive Pulmonary Embolism: The recommended dose is 100 mg administered by IV infusion over 2 hours

Is it safe for me to take Activase?

Activase is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication if you have any of the following.

  • Active internal bleeding
  • Recent intracranial or intraspinal surgery or serious head trauma
  • Intracranial conditions that may increase the risk of bleeding
  • Bleeding diathesis
  • Current severe uncontrolled hypertension
  • Current intracranial hemorrhage
  • History of recent stroke

Serious side effects are rare with Activase. Contact your healthcare provider immediately if you experience any of the following.

  • Severe bleeding
  • Swelling

Effects of other drugs, drug classes and over-the-counter products on Activase

The following medications may interact with Activase:

  • Anticoagulants and drugs that inhibit platelet function increase the risk of bleeding when administered with Activase therapy
  • Concomitant angiotensin-converting enzyme inhibitors may increase the risk of orolingual angioedema
