Upadacitinib is a JAK inhibitor and is used for the following:
- Rheumatoid arthritis (RA)
- Psoriatic arthritis (PsA)
- Eczema (atopic dermatitis)
- Ulcerative colitis (UC)
- Crohn’s disease (CD)
- Ankylosing spondylitis (AS)
- Non-radiographic axial spondyloarthritis (nr-axSpA)
How does Upadacitinib work?
Upadacitinib is a janus kinase (JAK) inhibitor. It blocks the JAK enzyme (protein) that plays a role in your immune system to cause inflammation. By blocking this enzyme, Upadacitinib is thought to calm down your immune system to relieve symptoms of arthritis and certain inflammatory conditions.
How do I take Upadacitinib?
Upadacitinib is typically dosed as follows:
- All approved types of arthritis (RA, PsA, AS, and nr-axSpA): The typical dose is 15 mg by mouth once a day
- Eczema: The typical dose is 15 mg by mouth once a day. If you’re between the ages of 12 and 65 and weigh at least 88 lbs, your provider might raise your dose to 30 mg once a day if you’re not seeing an improvement in your symptoms
- Ulcerative colitis: The typical starting dose is 45 mg by mouth once a day for 8 weeks. Then, the recommended maintenance dose is 15 mg once a day thereafter. But if you’re not seeing an improvement in your symptoms and your UC is severe, your provider might raise your maintenance dose to 30 mg once a day
- Crohn’s disease: The typical starting dose is 45 mg by mouth once a day for 12 weeks. Then, the recommended maintenance dose is 15 mg once a day thereafter. But if you’re not seeing an improvement in your symptoms and your CD is severe, your provider might raise your maintenance dose to 30 mg once a day
Your dose might differ if you have kidney or liver problems.

Upadacitinib Serious Side Effects
Serious side effects are rare with Upadacitinib. Contact your healthcare provider immediately if you experience any of the following.
- Serious infection: fever, shortness of breath, muscle aches, cough, weight loss, painful sores on your skin
- Blood clots: swelling and pain in your arms or legs, warm skin, trouble breathing
- Stroke: slurred speech, trouble walking, numbness on one side of your body or face, drooping face
- Heart attack: chest pain or tightness; pain that radiates up to your shoulder, arm, neck, or jaw
- Stomach tear: stomach pain, nausea, vomiting, blood in your stools
- Serious allergic reactions: hives; swelling of your lips, tongue, or throat; trouble breathing
Effects of other drugs, drug classes and over-the-counter products on Upadacitinib
The following medications may interact with Upadacitinib:
- Strong CYP3A4 Inhibitors: See the Full Prescribing Information for dosage modification for patients with atopic dermatitis, ulcerative colitis, and Crohn’s disease
- Strong CYP3A4 Inducers: Coadministration of Upadacitinib with strong CYP3A4 inducers is not recommended

Is Upadacitinib safe in pregnancy?
Based on findings in animal studies, Upadacitinib may cause fetal harm when administered to a pregnant woman. Verify the pregnancy status of females of reproductive potential prior to
starting treatment with Upadacitinib. If you can become pregnant, use effective contraception during treatment with Upadacitinib and for 4 weeks following completion of therapy.
