SENSIPAR

Sensipar (cinacalcet) is indicated for the treatment of secondary hyperparathyroidism (HPT) in adult patients with chronic kidney disease (CKD) on dialysis. Sensipar should not be used in patients with CKD who are not on dialysis due to an increased risk of low calcium levels.

Regarding the decision to refer patients for surgery, we know that untreated hyperparathyroidism increases the risk of fractures by 75%, cardiovascular disease by 250%, kidney stones by 500%, and dementia by 41%.

It is best to take this medicine with food or shortly after a meal. Swallow the tablet whole. Do not crush, break, or chew it.

Too much calcium in your blood can weaken your bones, create kidney stones, and interfere with how your heart and brain work. Hypercalcemia is usually a result of overactive parathyroid glands.

Most people with asymptomatic primary hyperparathyroidism can have a fairly benign course for years, even a lifetime. But hyperparathyroidism does progress in some people, and they should undergo surgery, which is the only complete cure.

Dizziness, diarrhea, and decreased appetite are other side effects of this medication as well. Cinacalcet can cause hypocalcemia. The patients on this medication need to be closely followed for common symptoms associated with hypocalcemia, including tingling, muscle twitching, cramps, changes in mood, or irritability.

Cinacalcet is used to treat hyperparathyroidism in patients with chronic kidney disease who are on dialysis. Hyperparathyroidism is a condition that is caused when the parathyroid glands located in the neck make too much parathyroid hormone (PTH).

In primary hyperparathyroidism, one or more of the parathyroid glands is overactive. As a result, the gland makes too much parathyroid hormone (PTH). Too much PTH causes calcium levels in your blood to rise too high, which can lead to health problems such as bone thinning and kidney stones.

Cinacalcet comes as a tablet to take by mouth. It is usually taken once a day with food or shortly after a meal. To help you remember to take cinacalcet, take it at around the same time every day.

Sensipar is the trade name, the generic name for this drug is Cinacalcet. Sensipar comes as a small pill in three strengths (30mg, 60mg, and 90mg).

Because cinacalcet and vitamin D act through distinct mechanisms, their combined application could lead to more effective control of PTH levels, and their offsetting effects on calcium and phosphate may reduce the risks of hypercalcemia and hyperphosphatemia that are increased by vitamin D monotherapy (13,18).

The other unique contribution of our case is showing that cinacalcet increased urinary calcium excretion. We propose that this is due to activation of renal CaSRs. CaSRs are most highly expressed in the thick ascending limb of Henle.

Clinical studies have shown that PTH response to cinacalcet is rapid, with nadir PTH concentration occurring approximately 3 hours after dosing; PTH concentrations remain suppressed at 24 hours as compared with baseline. Cinacalcet half-life is about 6 hours, and the terminal half-life ranges from 30 to 40 hours.

Cinacalcet does not affect longitudinal growth but increases body weight gain in experimental uraemia.

Within a year of halting cinacalcet therapy, patients with secondary hyperparathyroidism experience rises in parathyroid hormone (PTH), serum calcium, and primary calciprotein 1 (CCP-1) particles, according to findings from a real-world study.

When you take Sensipar, it works on your parathyroid glands. It causes them to release less PTH. And when your PTH level goes down, less calcium and phosphorus are released from your bones.

Relation to Diet: Avoid or limit high-phosphorus foods as advised by your doctor and dietitian. High-phosphorus foods include dairy products (milk, yogurt, cheese, pudding, ice cream), chocolate, nuts, peanuts, peanut butter, bran cereal, bran muffins, legumes (chickpeas, kidney beans, navy beans).

Secondary efficacy parameters also improved in patients treated with Sensipar. These studies showed that Sensipar reduced PTH while lowering Ca x P, calcium, and phosphorus levels (Table 1, Figure 2).

Many patients actually get worsening kidney function taking Sensipar. Most insurance companies will not pay for this drug for treating primary hyperparathyroidism.

Low blood pressure: Blood pressure may occasionally drop when taking cinacalcet. Be alert for any lightheadedness, dizziness, or a faster-than-normal heart rate. Move slowly when moving from a lying to an upright position. If your blood pressure drops too low, your doctor may adjust your dose or try another medication.

Cinacalcet lowers the amount of PTH which lowers the calcium and phosphorus concentrations. Cinacalcet is also used to lower calcium in the blood of patients with parathyroid cancer and primary hyperparathyroidism. This medicine is available only with your doctor’s prescription.

Cinacalcet is a positive allosteric modulator of the calcium sensing receptor (CaSR) that increases the sensitivity of the CaSR on the parathyroid glands, thereby reducing PTH secretion and serum Ca levels . Cinacalcet is widely used in patients with secondary or tertiary hyperparathyroidism.

Nausea, vomiting, or unusual tiredness may occur. Cinacalcet can cause low calcium blood levels. Tell your doctor right away if you have any serious side effects, including: black/bloody stools, stomach/abdominal pain, vomit that contains blood or looks like coffee grounds.

High phosphorus often does not cause symptoms itself. However, extra phosphorus in your blood can remove calcium from your bones and other parts of your body, leading to low calcium (also called hypocalcemia). Low calcium does cause symptoms, such as: Muscle cramps.

Side Effects: In clinical trials, the most common side effects reported in patients were nausea, vomiting, and diarrhea.

Cinacalcet lowers parathyroid hormone levels and improves calciumphosphorus homeostasis in patients receiving hemodialysis who have uncontrolled secondary hyperparathyroidism.

One or more of the parathyroid glands produces high amounts of parathyroid hormone. This leads to high calcium levels and low phosphorus levels in your blood. Primary hyperparathyroidism usually occurs randomly. But some people inherit a gene that causes the disorder.

Calcitriol (Rocaltrol) comes as capsules and a liquid that you take by mouth. It’s available as a brand-name and generic medication. Cinacalcet (Sensipar) lowers high parathyroid hormone levels in people with overactive parathyroid glands caused by different medical conditions, including parathyroid cancer.

Treatment with activated vitamin D, or calcitriol [1,25(OH) 2D 3], has been used for over three decades to manage the secondary hyperparathyroidism and hypocalcemia accompanying chronic renal disease.

Calcitriol, which is actually the most active form of vitamin D [100-times higher than 25(OH)D3] has a very short half-life, of about a few hours. While calcitriol was first thought to be generated solely in the kidney, it is now recognized in many extrarenal tissues.

Oral calcitriol effectively decreases PTH levels, decreases bone resorption, improves endosteal fibrosis and mineralization, and to some extent helps in the bone pains associated with renal osteodystrophy. Calcitriol has a direct inhibiting effect on PTH synthesis.

A recent publication of the observational cohort Health, Aging, and Body Composition (Health ABC) Study reports that lower calcitriol levels are independently associated with kidney function decline.

Calcitriol is used to treat and prevent low levels of calcium and bone disease in patients whose kidneys or parathyroid glands (glands in the neck that release natural substances to control the amount of calcium in the blood) are not working normally.

Calcitriol is a hormonally-active, synthetic vitamin D analog prescribed to treat hypocalcemia, osteoporosis, and the prevention of corticosteroid-induced osteoporosis.

Conclusion: Cinacalcet combined with low-dose calcitriol can improve high calcium, high phosphorus, and high iPTH in MHD patients with severe SHPT and also improve bone metabolism. It can be used as a favorable choice for SHPT treatment.

By the early 1970s, it had become clear that calcitriol was the active form of vitamin D. It is over 1,000 times as potent as Vitamin D2 and D3 in binding to the vitamin D receptor.
