pemetrexed is an antimetabolite and is used for the following:
- Advanced or metastatic non-squamous non-small cell lung cancer (NSCLC)
- Advanced or metastatic malignant mesothelioma
How does pemetrexed work?
pemetrexed is a type of chemotherapy called an antimetabolite. It stops rapidly growing cells (like cancer cells) from producing the chemicals needed to build their genetic material and other substances important for cells to grow and divide.
How do I take pemetrexed?
- Your treatment team will determine your pemetrexed dose based on your body surface area (BSA), which is calculated using your height and weight
- The typical dose is 500 mg/m2 given as an infusion through the vein (IV) every 3 weeks by a healthcare provider
Is it safe for me to take pemetrexed?
pemetrexed is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you.
pemetrexed Common Side Effects
Common side effects of pemetrexed:
- Low blood cell counts
- Higher blood sugar levels
- Feeling very tired
- Nausea
- Signs of stress to the liver and kidney on blood tests
- Constipation
- Low blood electrolyte and protein levels
- Diarrhea
pemetrexed Serious Side Effects

- Lower blood cell counts: unusual bleeding or bruising, fever, chills, body aches, tiredness
- Serious kidney problems: feeling confused, tiredness, nausea, vomiting, itchy skin, making little to no urine
- Severe skin problems: burning, redness, rashes, blisters, or peeling skin
- Lung damage: cough, trouble catching your breath, fever
Effects of other drugs, drug classes and over-the-counter products on pemetrexed
The following medications may interact with pemetrexed:
- Ibuprofen increased risk of pemetrexed toxicity in patients with mild to moderate renal impairment. Modify the ibuprofen dosage as recommended for patients with a creatinine clearance between 45 mL/min and 79 mL/min

Based on findings from animal studies and its mechanism of action, pemetrexed can cause fetal harm when administered to a pregnant woman. Women should use effective contraception during treatment with pemetrexed and for 6 months after the last dose. Males with female partners of reproductive potential to use effective contraception during treatment with pemetrexed and for 3 months after the last dose.
