INSPRA is a mineralocorticoid (aldosterone) antagonist. Aldosterone is a natural hormone in the body that normally raises your blood pressure by increasing the amount of salt and water in your bloodstream. So by blocking aldosterone, INSPRA helps your body get rid of extra salt and water, which then lowers your blood pressure. It’s unclear how exactly INSPRA helps with heart failure. But it’s thought that the medication helps protect the heart. This will help reduce the risk of death in people with heart failure.

INSPRA is typically dosed as 25 mg to 50 mg by mouth once daily. The max dose is 50 mg by mouth once or twice daily, depending on the condition being treated.

INSPRA is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication if you have a serum potassium >5.5 mEq/L at initiation, creatinine clearance ≤30 mL/min, have type 2 diabetes with microalbuminuria, a serum creatinine >2.0 mg/dL in males, >1.8 mg/dL in females, a creatinine clearance <50 mL/min with hypertension, or are taking a strong CYP3A inhibitor, potassium supplement, or potassium-sparing diuretic.

  • CYP3A Inhibitors: In post-MI HFrEF patients do not exceed 25 mg once daily when used with moderate CYP3A inhibitors (e.g., verapamil, erythromycin, saquinavir, fluconazole). In patients with hypertension initiate at 25 mg once daily. For inadequate blood pressure response, dosing may be increased to a maximum of 25 mg twice daily.
