Cladribine is an antimetabolite used to treat relapsing forms of multiple sclerosis (MS).

Cladribine is a purine antimetabolite. It is not fully understood how it works, but it’s thought to treat MS by killing lymphocytes, a type of white blood cell (WBC). Normally, WBCs protect your body from infections. But in people with MS, they can attack your nerves and cause inflammation (swelling). So Cladribine treats MS by killing these WBCs to protect your nerves.

Your total dose of Cladribine depends on your weight (3.5 mg/kg). The total dose isn’t taken all at once, but each treatment course is divided in half and taken about 1 year apart. Each treatment course is also divided into two treatment cycles, about 1 month apart.

Cladribine is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication in the following:

• Cancer
• Human immunodeficiency virus (HIV)
• Active long-term infections, such as hepatitis or tuberculosis
• Currently pregnant
• Child-bearing age (age when you’re able to have children) and not planning to use effective birth control during and for 6 months after Cladribine treatment
• Breastfeeding during and for 10 days after the last dose of Cladribine treatment

Common side effects of Cladribine:

• Upper respiratory tract infections
• Low lymphocyte count
• Nausea
• Back pain
• Muscle and joint pain
• Trouble sleeping

Serious side effects are rare with Cladribine. Contact your healthcare provider immediately if you experience any of the following:

• Cancer
• Serious infections: fever, feeling unwell, low appetite, muscle aches or pain, blistered rash or skin blotches, burning, tingling, itching in the skin, vision changes, thinking problems, confusion, problems with balance, weakness on one side of your body
• Blood problems: dizziness, tiredness, weakness, bruising more easily, bleeds that are difficult to stop
• Liver injury: nausea, vomiting, stomach pain, fatigue, low appetite, yellowing skin, dark urine
• Serious allergic reactions: rash, mouth ulcers, throat swelling, headache, double vision
• Heart failure: shortness of breath, fast heart beat, swelling or fluid buildup in your body

The following medications may interact with Cladribine:

• Immunosuppressive drugs
• Hematotoxic drugs
• Antiviral and antiretroviral drugs

Cladribine may harm your unborn baby. Female patients who can become pregnant should use effective contraception while on this medication and for at least 6 months after the last dose in each treatment course. Male patients with partners that can become pregnant should also use effective contraception while on this medication and for at least 6 months after the last dose in each treatment course. If you or your partner become pregnant while on Cladribine, notify your doctor immediately.
