POLIVY is the brand name for polatuzumab vedotin-piiq IV infusion.
What dosages is POLIVY available in?
POLIVY is available as a 30 mg or 140 mg single-dose vial.
What is POLIVY used for?
POLIVY is a CD79b-directed antibody and microtubule inhibitor conjugate indicated: In combination with a rituximab product, cyclophosphamide, doxorubicin, and prednisone (R-CHP) for the treatment of adult patients who have previously untreated diffuse large B-cell lymphoma (DLBCL), not otherwise specified (NOS) or high-grade B-cell lymphoma (HGBL) and who have an International Prognostic Index score of 2 or greater. In combination with bendamustine and a rituximab product for the treatment of adult patients with relapsed or refractory DLBCL, NOS, after at least two prior therapies.
How does POLIVY work?
POLIVY works by blocking a protein that causes cancer cells to grow and multiply. This helps to slow or stop the spread of cancer cells.
How do I take POLIVY?
The recommended dose of POLIVY is 1.8 mg/kg as an intravenous infusion every 21 days for 6 cycles.

POLIVY is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you.

Common side effects of POLIVY:
- Diarrhea
- Dizziness
- Loss of appetite
- Vomiting

POLIVY Serious Side Effects
Serious side effects are rare with POLIVY. Side effects that you should report to your care team as soon as possible:
- Allergic reactions—skin rash, itching, hives, swelling of the face, lips, tongue, or throat
- Dizziness, loss of balance or coordination, confusion or trouble speaking
- Infection—fever, chills, cough, sore throat, wounds that don’t heal, pain or trouble when passing urine, general feeling of discomfort or being unwell
- Infusion reactions—chest pain, shortness of breath or trouble breathing, feeling faint or lightheaded
Liver injury—right upper belly pain, loss of appetite, nausea, light-colored stool, dark yellow or brown urine, yellowing skin or eyes, unusual weakness or fatigue - Low red blood cell level—unusual weakness or fatigue, dizziness, headache, trouble breathing
- Pain, tingling, or numbness in the hands or feet
- Stomach pain, unusual weakness or fatigue, nausea, vomiting, diarrhea, or fever that lasts longer than expected
- Tumor lysis syndrome (TLS)—nausea, vomiting, diarrhea, decrease in the amount of urine, dark urine, unusual weakness or fatigue, confusion, muscle pain or cramps, fast or irregular heartbeat, joint pain
- Unusual bruising or bleeding
Effects of other drugs, drug classes and over-the-counter products on POLIVY
The following medications may interact with POLIVY:
- Concomitant use of strong CYP3A inhibitors or inducers has the potential to affect the exposure to unconjugated monomethyl auristatin E (MMAE)

Is POLIVY safe in pregnancy?
POLIVY can cause harm to your unborn baby when administered during pregnancy. You should take a pregnancy test prior to the initiation of POLIVY. Females of reproductive potential to use effective contraception during treatment and for
3 months following the last dose of POLIVY.
