Orforglipron (the active ingredient). Foundayo belongs to the same class of medicines, GLP-1 receptor agonists, as semaglutide (Wegovy tablets and Wegovy injection) and tirzepatide (Mounjaro injection). Unlike those large-peptide medicines, orforglipron is a small molecule, which is what allows it to work as a daily tablet.

About Foundayo

Foundayo is the brand name for orforglipron, a once-daily oral GLP-1 medicine developed for weight loss. It is designed to help adults living with obesity, or who are overweight and have a weight-related health condition, to lose weight and keep it off alongside a healthy diet and increased physical activity.

What sets orforglipron apart from other GLP-1 medicines is its structure. It is a small molecule rather than the large peptide found in semaglutide or tirzepatide, and this smaller molecular size is what allows it to be absorbed and taken as a daily tablet rather than an injection. Crucially, unlike the oral form of Wegovy, which must be taken on an empty stomach followed by a 30-minute fast, Foundayo can be taken with or without food, making it far easier to fit into your day.

Foundayo is a prescription-only medicine. It is still subject to UK regulatory approval, and once approved, NowPatient intends to offer Foundayo as a treatment for weight management following a video consultation with a weight management clinician.

Key facts of Foundayo

  • A once-daily oral GLP-1 tablet for weight management in adults with obesity, or overweight with a weight-related health condition.
  • Can be taken with or without food, unlike the Wegovy tablet, which needs an empty stomach and a 30-minute fast.
  • Made by Eli Lilly; the active ingredient is orforglipron, a small-molecule GLP-1 receptor agonist.
  • Most side effects are digestive (nausea, vomiting, diarrhoea, constipation) and are usually mild to moderate, mainly during dose escalation.
  • It can interact with other medicines, including statins (simvastatin) and oral contraceptives. Tell your prescriber everything you take.
  • Foundayo is not yet licensed in the UK. It is approved in the United States and is under MHRA review, expected later in 2026.
  • It should be used alongside a reduced-calorie diet and increased physical activity.

Who is Foundayo suitable for?

Foundayo is intended for adults aged 18 and over who are living with obesity, or who are overweight and have a weight-related health condition. Eligibility for a GLP-1 medicine through NowPatient’s weight management service is generally based on body mass index (BMI). This is typically a BMI of 30 or above, or a BMI of 27 or above alongside at least one weight-related condition such as high blood pressure, raised cholesterol, fatty liver disease, sleep apnoea or PCOS.

Who is Foundayo not suitable for?

Foundayo is not suitable for everyone. Speak to the NowPatient prescribing clinician before using Foundayo if you:

  • Have a personal or family history of medullary thyroid cancer (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Foundayo must not be used in these cases because of a risk of thyroid C-cell tumours.
  • Have had a serious allergic (hypersensitivity) reaction to orforglipron or any of the ingredients in Foundayo.
  • Are pregnant, planning a pregnancy or breastfeeding. GLP-1 medicines are generally not recommended in these situations because of limited safety data; Foundayo should be stopped in pregnancy and is not recommended while breastfeeding.
  • Have a history of pancreatitis, gallbladder problems or kidney problems.
  • Have type 2 diabetes with diabetic retinopathy, which may need closer monitoring.
  • Take insulin or a sulfonylurea, which can increase the risk of low blood sugar.
  • Are scheduled for surgery or a procedure involving anaesthesia or deep sedation, because delayed stomach emptying raises the risk of food or liquid entering the lungs.

How and where to get Foundayo?

Foundayo is not yet available in the UK. It is currently approved in the United States only and is awaiting regulatory approval from the MHRA, which we expect at some point later in 2026. Until it is licensed and available here, we are not able to prescribe or supply it.

You can register your interest with NowPatient to be kept up to date on UK availability and pricing as soon as we have it. Once Foundayo is licensed, you will be able to book a free online video consultation with a NowPatient weight management clinician, who will assess whether it is a suitable treatment for you. If it is, a prescription will be issued and the medication dispensed and delivered from our registered pharmacy.

Dosage and strength for Foundayo for weight loss

Foundayo tablets contain orforglipron and are expected to come in six strengths: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg. The dose is increased gradually to help your body adjust and to reduce side effects such as nausea.

  • Starting dose: 0.8 mg once daily.
  • Escalation: the dose is stepped up about every 30 days, as tolerated and on your clinician’s advice.
  • Maintenance dose: up to a maximum of 17.2 mg once daily.

How to take it:

  • Take one tablet once daily, with or without food, at roughly the same time each day.
  • Swallow the tablet whole. Do not break, crush or chew it.
  • Many people start to notice weight loss within the first few weeks, with further dose-dependent loss over time.

Missed dose: if you miss a dose, take the next one as soon as you can, but never take two doses to make up for a missed one. If you miss 7 or more doses, you may need to restart at a lower dose to reduce side effects, so check with your prescriber for guidance.

What happens if you use too much Foundayo?

Taking more than the prescribed amount will not speed up weight loss and can increase the risk of side effects such as nausea, vomiting, diarrhoea and low blood sugar. If you have taken too much and feel unwell, speak to your doctor, contact NHS 111, or reach out to one of our prescribing clinicians.

What are the side effects of Foundayo?

Foundayo’s side effects are similar to those of other GLP-1 medicines and are generally mild to moderate.

Common side effects (mostly digestive):

  • Nausea
  • Vomiting
  • Diarrhoea
  • Constipation
  • Indigestion, tummy (abdominal) pain, wind, bloating, burping or heartburn
  • Headache
  • Tiredness (fatigue)
  • Hair loss

Less common but more serious effects. Speak to your NowPatient weight management clinician if you notice:

  • Signs of a thyroid tumour, such as a lump or swelling in the neck, hoarseness, trouble swallowing or shortness of breath
  • Severe, persistent pain in your stomach area, which may spread to the back (possible pancreatitis)
  • Nausea, vomiting or diarrhoea that does not go away, which can cause dehydration and, rarely, kidney problems
  • Low blood sugar, especially if you also take insulin or a sulfonylurea
  • Changes in vision (in people with type 2 diabetes)
  • Signs of gallbladder problems, such as upper stomach pain, fever, yellowing of the skin or eyes, or clay-coloured stools

Stop taking Foundayo and seek medical help straight away if you have signs of a serious allergic reaction: swelling of the face, lips, tongue or throat; difficulty breathing or swallowing; a severe rash or itching; fainting; dizziness; or a rapid heartbeat. Call 999 if symptoms are severe.

Because GLP-1 medicines slow stomach emptying, there is an increased risk of food or liquid entering the lungs during surgery or procedures involving anaesthesia or deep sedation. Tell everyone involved in your care that you take Foundayo before any planned procedure.

These are not all the possible side effects. Talk to your healthcare provider about any side effects that bother you, get worse or do not go away. When Foundayo is available in the UK, suspected side effects can be reported through the MHRA Yellow Card Scheme at yellowcard.mhra.gov.uk or directly on the NowPatient app.

Information about using Foundayo with other medications

Because Foundayo slows stomach emptying, it can affect how other medicines you take by mouth are absorbed. Tell your prescribing clinician about everything you take, including prescription medicines, over-the-counter products, vitamins and herbal supplements. Key points:

  • Simvastatin: do not exceed simvastatin 20 mg once daily while taking Foundayo.
  • Oral contraceptives (the pill): GLP-1 medicines may reduce the effectiveness of oral hormonal contraceptives. If you use the pill, add a non-oral or barrier method of contraception for 4 weeks after starting Foundayo and for 4 weeks after each dose increase.
  • Insulin and sulfonylureas: increased risk of low blood sugar; dose adjustments may be needed.

Why might Foundayo not work for weight loss?

The amount of weight you might lose depends on your dose, how consistently you take the medicine and your individual health. Foundayo works best when taken every day as prescribed, with the dose increased gradually to the maintenance level, and combined with a reduced-calorie diet and increased physical activity. Weight loss occurs over months rather than days. If your weight has not improved after reaching and maintaining your target dose, speak with your healthcare provider. Results vary between individuals.

Can Foundayo be used long-term?

Yes. Treatment usually begins at a low dose that is increased gradually for effective weight loss, with the first few months allowing your body to adjust to the medicine and to lifestyle changes. In clinical trials, people on the highest dose lost an average of between 11% and 12.4% of their body weight (around 12.4 kg) over 72 weeks, alongside a healthy diet and exercise. Weight loss tends to be maintained for as long as you keep up healthy habits and continue the medicine as prescribed. Long-term use should always be monitored by a healthcare professional.

How to store Foundayo

Full storage instructions for the UK will be confirmed in the patient information leaflet once Foundayo is licensed and available here. As with all medicines, you should keep Foundayo out of the sight and reach of children, follow the storage advice on the packaging and leaflet, and not use it after the expiry date shown. Do not dispose of unused medicine in household waste or wastewater; ask your pharmacist how to dispose of medicines you no longer use.

Is there a herbal Foundayo?

No. There is no herbal version of Foundayo. If you have been offered such a product, it may be counterfeit or unsafe. Orforglipron is a prescription-only medicine and should only ever be obtained from a registered pharmacy following a prescription.

Should I avoid any particular food or drink when using Foundayo?

One of the advantages of Foundayo is that it can be taken with or without food, so there is no fasting requirement and no need to time it around meals, unlike the Wegovy tablet. You should still follow a reduced-calorie diet as part of your weight-management plan. Eating smaller meals and avoiding very fatty or rich foods can help reduce nausea, particularly during dose escalation. If you take simvastatin, do not exceed 20 mg once daily while on Foundayo.

What lifestyle changes can I make to help with weight loss?

Foundayo works best alongside healthy lifestyle changes:

  • Follow a reduced-calorie diet as recommended by your healthcare provider.
  • Increase physical activity, aiming for at least 150 minutes of moderate exercise per week.
  • Eat more vegetables, fruit, whole grains and lean proteins.
  • Reduce sugary drinks, processed foods and high-fat foods.
  • Get adequate sleep (7 to 9 hours per night).
  • Manage stress through relaxation techniques.
  • Stay hydrated by drinking plenty of water, which also helps reduce the risk of dehydration if you experience vomiting or diarrhoea.

Switching from Wegovy or Mounjaro to Foundayo

Yes, switching from another GLP-1 such as Wegovy or Mounjaro to Foundayo is possible, and studies suggest people maintain the weight they have lost after switching. Any change should be made only with your clinician’s guidance and will need a new prescription. Your starting dose depends on your current or most recently used dose:

  • Switching from an injection (Wegovy or Mounjaro): start Foundayo one week after your last injection.
  • Switching from the Wegovy tablet: you can start Foundayo the day after stopping it.

How is Foundayo different from Wegovy and Mounjaro?

Foundayo, Wegovy and Mounjaro are all GLP-1 based weight-loss medicines, but they differ in licensing, how they are taken and their molecular structure. The table below summarises the main differences.

Wegovy is also licensed to reduce the risk of major adverse cardiovascular events, and the injection is approved for non-cirrhotic metabolic dysfunction-associated steatohepatitis (MASH). Mounjaro is approved to treat moderate-to-severe obstructive sleep apnoea in adults with obesity. With any of these medicines, the dose is increased gradually (usually after about 28 to 30 days) based on how well you respond and tolerate it.

Alternatives to Foundayo

While Foundayo awaits UK approval, licensed alternatives for the treatment of obesity or overweight include:

Oral treatments: Wegovy tablets (semaglutide); Orlistat (Xenical / Alli); and Mysimba (naltrexone/bupropion).

Injectable treatments: Wegovy injection (semaglutide), Mounjaro (tirzepatide) and Saxenda (liraglutide).

FAQs

Answers to Popular Questions About Foundayo

What is Foundayo?

What is Foundayo?

Foundayo is the brand name for orforglipron, a once-daily oral GLP-1 receptor agonist developed by Eli Lilly for chronic weight management in adults. It is taken as a tablet, with or without food, alongside a reduced-calorie diet and increased physical activity. Foundayo is approved in the United States and is awaiting MHRA review in the UK.

How does Foundayo work?

Foundayo mimics a naturally occurring hormone called GLP-1, which helps regulate blood sugar, appetite and metabolism. It slows the rate at which your stomach empties, so you feel fuller for longer, and acts on appetite signals in the brain to reduce how much you want to eat. Combined with dietary changes and exercise, this can lead to meaningful weight loss. Because orforglipron is a small molecule rather than a large peptide, it can be taken as a daily tablet rather than an injection.

Can I take Foundayo with food?

Yes. Unlike the Wegovy tablet, which must be taken on an empty stomach with a 30-minute fast before eating or drinking, Foundayo can be taken with or without food at roughly the same time each day. Swallow it whole and do not break, crush or chew it.

How effective is Foundayo for weight loss?

In clinical trials, adults living with obesity or overweight lost an average of between 11% and 12.4% of their body weight (around 12.4 kg) after 72 weeks on the highest dose, alongside a healthy diet and regular exercise. The amount of weight you might lose depends on your dose, how consistently you take the medicine and your individual health.

How do I take Foundayo?

Take one tablet once daily, with or without food, at roughly the same time each day, and swallow it whole. Treatment usually starts at 0.8 mg daily and is increased step by step, about every 30 days as tolerated and on your clinician’s advice, up to a maximum maintenance dose of 17.2 mg. If you miss a dose, take the next one as soon as you can, but never take two doses at once.

What are the most common side effects?

The most common side effects are digestive: nausea, vomiting, diarrhoea, constipation, indigestion, abdominal pain, wind, bloating, burping or heartburn. Headache, tiredness and hair loss can also occur. These are usually mild to moderate and most common during dose escalation.

Can I switch from Wegovy or Mounjaro to Foundayo?

Yes, with your clinician’s guidance and a new prescription. If switching from an injection, you would start Foundayo one week after your last injection. If switching from the Wegovy tablet, you can start Foundayo the day after stopping it. Studies suggest people maintain the weight they have lost after switching.

Is Foundayo available in the UK, and how much will it cost?

Not yet. Foundayo is currently approved in the United States only and is awaiting MHRA approval in the UK, which we expect at some point later in 2026. Until it is licensed and available here, we are not able to prescribe or supply it, and UK pricing has not been confirmed. You can register your interest with NowPatient to be kept up to date on availability and pricing.

How often will I check in with a clinician?

Under NowPatient’s weight management programme, patients check in monthly at first so clinicians can fine-tune the dose for the best results. Once your weight loss settles, check-ins usually become less frequent, at around every 3 to 6 months. You can also request an on-demand weight management video consultation with a NowPatient clinician, available 7 days a week, if you need to discuss side effects, switching medicines or dosing.
