Vagifem is classed as a hormone replacement therapy (HRT). It contains estradiol, which acts in the same way as the female sex hormone oestrogen. Can Vagifem used for HRT be used long-term?

HRT should only be used as a short-term treatment for the menopause. Women who use HRT for more than 5 years may be at an increased risk of ovarian cancer. Speak to your GP to review your treatment at least once each year and have regular checkups.

Is there a herbal Vagifem?

No. Herbal Vagifem does not exist. If you have been marketed such a formulation, it may be fake.

What is Vagifem?

Vagifem is a vaginal tablet containing oestradiol 10 micrograms, used to treat vaginal atrophy (dryness, itching, discomfort, and painful sex) caused by oestrogen deficiency after the menopause. Each small tablet is inserted into the vagina using an applicator. Vagifem provides local oestrogen replacement with minimal systemic absorption, making it very safe for long-term use. How does Vagifem work?

Vagifem delivers a tiny dose of oestradiol directly to the vaginal tissues, restoring the thickness, moisture, and elasticity of the vaginal wall. It normalises vaginal pH, reduces the risk of vaginal infections, and can improve urinary symptoms such as urgency and recurrent UTIs. Because the dose is so low and local, systemic absorption is negligible.

How do I use Vagifem?

Insert one Vagifem tablet into the vagina at bedtime using the single-use applicator. For the first 2 weeks, use one tablet daily (initial phase). After 2 weeks, reduce to one tablet twice weekly (maintenance). Do not use more frequently than recommended. The applicator is for single use only — dispose of it after each insertion.

How long should I use Vagifem?

Vagifem is intended for long-term use, as vaginal atrophy is a chronic condition and symptoms return when treatment is stopped. There is no time limit on vaginal oestrogen use. Annual reviews with your prescriber are recommended but treatment can continue indefinitely if it remains beneficial.

What are the side effects of Vagifem?

Side effects are uncommon. Local vaginal irritation, itching, or burning may occur when first starting, usually settling within a few days. Very rare systemic effects include breast tenderness and headache. Vagifem does not carry the risks associated with systemic HRT and does not require progestogen addition, even in women with an intact uterus.

Is Vagifem safe for breast cancer survivors?

Vaginal oestrogen is generally considered acceptable for breast cancer survivors with vaginal atrophy, although the decision should be made in consultation with your oncologist. BMS and NICE guidance supports the cautious use of vaginal oestrogen when non-hormonal treatments have been tried. Vagifem’s ultra-low dose (10 mcg) has negligible systemic absorption.
