FAQs
Answers to popular questions about Premarin
What is Premarin?
Premarin is an oestrogen-only hormone replacement therapy (HRT) containing conjugated equine oestrogens (CEE) 0.3 mg, 0.625 mg, or 1.25 mg. Conjugated oestrogens are derived from the urine of pregnant mares and contain a mixture of oestrogen compounds. Premarin has been used for menopausal symptom relief since the 1940s and is one of the most studied HRT products. It is used to treat vasomotor symptoms (hot flushes, night sweats), vulvovaginal atrophy, and osteoporosis prevention in postmenopausal women.
How does Premarin differ from other oestrogen HRT?
Premarin contains conjugated equine oestrogens, which is a mixture of oestrogen compounds rather than a single molecule like oestradiol (used in Elleste Solo, Evorel, and Femoston). Modern prescribing guidelines tend to favour body-identical oestradiol-based HRT products, as oestradiol is molecularly identical to the oestrogen produced by the human ovary. However, Premarin remains licensed and effective, and some women prefer it, particularly those who have used it successfully for years.
Who can take Premarin?
Premarin is used for postmenopausal women with moderate to severe vasomotor symptoms or vulvovaginal atrophy. As an oestrogen-only product, it is most appropriate for women who have had a hysterectomy. Women with an intact uterus must take a progestogen alongside Premarin (or use Premique, which combines CEE with medroxyprogesterone acetate). Premarin should not be used in women with active breast cancer, undiagnosed vaginal bleeding, active VTE, or severe liver disease.
How do I take Premarin?
Take one Premarin tablet daily at the same time, continuously without a break. The usual starting dose is 0.625 mg, but your prescriber may start at a lower dose (0.3 mg) and adjust based on symptom response. Swallow the tablet whole with water. It can be taken with or without food.
How long should I take Premarin?
HRT duration should be reviewed annually. Use the lowest effective dose for the shortest duration needed to control symptoms. Some women take HRT for many years if the benefits outweigh the risks. NICE does not recommend an arbitrary time limit. If you wish to stop, your prescriber may suggest tapering the dose gradually.
What are the side effects of Premarin?
Common side effects include breast tenderness, headaches, nausea, bloating, fluid retention, and leg cramps. Breakthrough bleeding may occur. Less common effects include mood changes, changes in libido, and skin reactions. These often improve within the first 3 months of treatment.
What are the risks of taking Premarin?
Oral conjugated oestrogens are associated with a small increased risk of VTE (blood clots), stroke, and — when combined with progestogen — breast cancer. These risks increase with age and duration of use. Premarin was the oestrogen used in the Women’s Health Initiative study. Transdermal oestradiol (patches, gels) may have a lower VTE and stroke risk than oral oestrogens. Discuss your individual risk profile with your prescriber.
Is Premarin still recommended?
Premarin remains licensed and available, but modern UK prescribing guidelines from NICE and the British Menopause Society (BMS) tend to favour body-identical oestradiol-based HRT. Oestradiol is molecularly identical to human oestrogen and is available in a wider range of formulations (tablets, patches, gels). However, if you are stable and well on Premarin, there is no automatic need to switch. Discuss options with your prescriber.
How can I get Premarin?
Premarin is available on NHS prescription from your GP, menopause specialist, or through online services like NowPatient. Your prescriber may discuss whether body-identical oestradiol-based HRT might be more appropriate for you. NowPatient offers online consultations and home delivery.
