Step-by-step guide to using in-app
Report Hospital Admissions is a proactive health monitoring service that helps you track your hospital visits and stays while contributing to wider medical research.
Every three months, we will prompt you to report any hospital admissions you may have experienced. This regular check-in creates a comprehensive record of your healthcare utilisation, helping you understand patterns in your health and communicate more effectively with your healthcare providers.
The reporting process is designed to be quick and straightforward. You will be asked about when you were admitted, the reason for your admission, how long you stayed, and the outcome of your visit. This information is securely stored in your personal health record and can be reviewed at any time. Your contributions support healthcare planning, medical research, and health system improvements, while also providing you with personalised insights into your health journey over time.
You are also able to make ad-hoc reports, if needed.
The service is free for all registered NowPatient users.
Hospital Admissions Reporting benefits both you and the wider healthcare community by creating a comprehensive understanding of healthcare needs and outcomes.
Track your health patterns and trends over time. Keep a detailed record of when and why you have needed hospital care. This creates a comprehensive health timeline that helps you and your healthcare providers identify patterns, risk factors, and opportunities for preventive care
Keep a complete record for your medical history. Build a thorough personal health record that documents your complete hospital care history. This becomes invaluable when seeing new doctors, managing chronic conditions, or coordinating care across multiple providers
Monitor your healthcare journey and hospital visits. By reporting regularly every three months, you can track whether your health is improving, stable, or requires additional attention. You might notice seasonal patterns, identify triggers for health issues, or see the positive effects of lifestyle changes or treatments
Receive personalised insights. Get customised feedback about your hospital admission patterns, including recommendations for preventive care, early warning signs to watch for, and strategies to help you stay healthier and avoid unnecessary hospitalisations
Better communicate with your healthcare providers. Armed with accurate hospital admission records, you can have more productive conversations with your healthcare providers. You will have precise dates, diagnoses, and outcomes readily available, making it easier to discuss your health history
Identify opportunities for prevention and early intervention. Understanding your hospital admission patterns helps you work with your healthcare team to implement preventive strategies. If you are being admitted frequently for a particular condition, your providers can adjust your treatment plan to help you stay healthier at home
Coordinate care more effectively. A complete hospital admission record helps all your healthcare providers stay informed about your health status, reducing the risk of conflicting treatments or missed information during transitions of care
Supports healthcare planning and resource allocation. Your reports help healthcare systems understand patient needs, plan staffing levels, allocate resources effectively, and ensure that hospitals are prepared to meet community health demands
Contributes to medical research and evidence-based healthcare. Real-world data from patients like you helps researchers understand disease progression, treatment effectiveness, and factors that lead to hospitalisation
Helps identify patterns in hospital admission trends. Aggregated data can reveal seasonal patterns, emerging health threats, gaps in outpatient care, and populations at high risk for hospitalisation, enabling healthcare systems to implement targeted interventions
Improves understanding of healthcare utilisation. By analysing why patients need hospital care, healthcare systems can develop better outpatient services, improve chronic disease management programmes, and create interventions that help patients stay healthy at home
Enhances healthcare services for all patients. The insights gained from hospital admissions reporting lead to better care coordination, improved discharge planning, more effective preventive care programmes, and reduced unnecessary hospitalisations
Identifies health disparities and access issues. Admission data helps healthcare systems identify communities or populations with inadequate access to preventive care or outpatient services, enabling targeted improvements in healthcare equity
