What is KALETRA?
KALETRA is the brand name for lopinavir and ritonavir and is available as an oral tablet and solution.
What dosages is KALETRA available in?
KALETRA is available in the following:
- Tablets: 200 mg lopinavir and 50 mg ritonavir
- Tablets: 100 mg lopinavir and 25 mg ritonavir
- Oral solution: 80 mg lopinavir and 20 mg ritonavir per milliliter
What is KALETRA used for?
KALETRA is a HIV ARV and is used to treat HIV infection (used with other antiretroviral medications).
How does KALETRA work?
KALETRA is a combination of two medications: lopinavir and ritonavir. They’re both protease inhibitors. They block a protein called protease from processing viral proteins that allow HIV to infect healthy cells. By blocking protease from working, KALETRA prevents HIV from making copies of itself and spreading. At low doses, such as in KALETRA, ritonavir helps slow the breakdown of lopinavir in the body. This “boosts” the levels of lopinavir in the body, which helps it work better and longer to fight the infection.
How do I take KALETRA?
Your dose can vary depending on your age, your health conditions, and your other medications (including other HIV medications). In general, KALETRA is taken by mouth either once daily or twice daily.
Adults
- Once-daily dosing: The typical dose is 800 mg/200 mg (lopinavir/ritonavir) by mouth once daily
- Twice-daily dosing: The typical dose is 400 mg/100 mg (lopinavir/ritonavir) by mouth twice daily
Children 2 weeks to 17 years of age
- Your child’s provider will determine their dose based on either their body weight or their body surface area (BSA). BSA is your child’s body size calculated based on weight and height
- KALETRA is typically taken by mouth twice daily. The maximum dose is 400 mg/100 mg (lopinavir/ritonavir) twice daily
Is it safe for me to take KALETRA?
KALETRA is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication in the following:
- Hypersensitivity to KALETRA (e.g., toxic epidermal necrolysis, StevensJohnson syndrome, erythema multiforme, urticaria, angioedema) or any of
its ingredients, including ritonavir - Co-administration with drugs highly dependent on CYP3A for clearance and for which elevated plasma levels may result in serious and/or lifethreatening events
Co-administration with potent CYP3A inducers where significantly reduced lopinavir plasma concentrations may be associated with the potential for loss of virologic response and possible resistance and cross resistance.
KALETRA Common Side Effects
Common side effects of KALETRA:
- Diarrhea
- Respiratory tract infection
- Nausea
- Feeling weak
- High cholesterol levels
- Vomiting
- Belly pain
- Muscle, joint, or back pain
- Headache or migraine
- Changes in taste
KALETRA Serious Side Effects
Serious side effects are rare with KALETRA. Contact your healthcare provider immediately if you experience any of the following.
- Severe allergic reactions: facial swelling, hives, rash, itchy skin, blistering or peeling of skin, body aches, trouble breathing
- Serious liver problems: nausea, vomiting, not feeling hungry, pain in the right side of the stomach, yellowing of the whites of the eyes or skin, dark-colored urine, light-colored stool
- Abnormal heart rhythm: chest pounding, trouble breathing, chest pain, feeling faint or lightheaded
Effects of other drugs, drug classes and over-the-counter products on KALETRA
The following medications may interact with KALETRA:
- Co-administration of KALETRA can alter the plasma concentrations of other drugs and other drugs may alter the plasma concentrations of lopinavir. The potential for drug-drug interactions must be considered prior to and during therapy

Is KALETRA safe in pregnancy?
Once daily KALETRA dosing is not recommended in pregnancy. Avoid use of KALETRA oral solution during pregnancy due to the alcohol content. There is a pregnancy exposure registry that monitors pregnancy outcomes in women exposed to KALETRA during pregnancy. Physicians are encouraged to register patients by calling the Antiretroviral Pregnancy Registry at 1-800-258-4263.
