The typical starting dose of ACTOPLUSMET is 15 mg/500 mg by mouth twice per day or 15 mg/850 mg (pioglitazone/metformin) by mouth once per day with meals. If needed, your physician will raise your dose slowly depending on how you respond to the medication.

  • Heart failure: shortness of breath, unusual swelling, tiredness, sudden weight gain
  • Lactic acidosis: fast breathing, fast or slow heartbeat, heartbeat that does not feel normal, very upset stomach or throwing up, feeling very sleepy, shortness of breath, feeling very tired or weak, dizziness, feeling cold, muscle pain
  • Bladder cancer: pain while you urinate, constant need to urinate, and blood in your urine
  • Liver problems: nausea or vomiting, stomach pain, unusual tiredness, loss of appetite, dark urine, yellowing of your skin or the whites of your eyes
  • Diabetic eye disease with swelling (macular edema): blurred vision, changes in vision

The following medications may interact with ACTOPLUS MET:

  • Strong CYP2C8 inhibitors such as gemfibrozil increase the levels and effects of pioglitazone. Topiramate and rifampin may decrease pioglitazone

  • Drugs that reduce metformin clearance such as ranolazine,vandetanib, dolutegravir, and cimetidine may increase the levels and effects of metformin.

  • Insulin secretagogues or insulin use may increase the risk for hypoglycemia
