Zestoretic is an ACE inhibitor/thiazide diurectic that is used to treat high blood pressure.
How does Zestoretic work?
Zestoretic is a combination of two medications that work together to lower blood pressure. Lisinopril is an angiotensin-converting enzyme (ACE) inhibitor. It relaxes your blood vessels by blocking a protein (ACE) that normally squeezes your blood vessels. Hydrochlorothiazide is a thiazide diuretic (water pill). It works in your kidneys to remove extra water and certain electrolytes (like sodium and potassium). This helps lower blood pressure.
How do I take Zestoretic?
Zestoretic is typically started at 10 mg/12.5 mg (lisinopril/hydrochlorothiazide) by mouth once daily. Your provider will adjust your dose based on how well it’s helping your blood pressure and if you’re having side effects. The maximum dose is 80 mg/50 mg (lisinopril/hydrochlorothiazide) once daily.

Zestoretic is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you.
You should not use this medication if you are hypersensitive to this product and have a history of angioedema related to previous treatment with an angiotensin converting enzyme inhibitor and in patients with hereditary or idiopathic angioedema. Because of the hydrochlorothiazide component, this product is contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived drugs.

Common side effects of Zestoretic:
- Dizziness
- Headache
- Cough
- Extreme tiredness
- Feeling lightheaded when standing after sitting or lying down
Zestoretic Serious Side Effects
Serious side effects are rare with Zestoretic. Contact your healthcare provider immediately if you experience any of the following.
- Angioedema (severe swelling): swelling of the face, arms or legs, lips, tongue, or throat
- Dangerously low blood pressure: dizziness, lightheadedness, pale skin, feeling faint
- Serious kidney problems: urinating less often than usual, swelling in your feet, ankles, or hands, unexplained weight gain
- Liver problems: yellowing of the skin or eyes, stomach pain, nausea, vomiting, confusion
- High blood potassium levels: nausea, muscle weakness, abnormal heartbeat
- Severe allergic reaction: swelling of your lips, tongue, throat, or face, trouble breathing, rash
Effects of other drugs, drug classes and over-the-counter products on Zestoretic
The following medications may interact with Zestoretic:
- Diuretics: Excessive drop in blood pressure.
- Antidiabetics: Increased risk of hypoglycaemia.
- NSAIDS: Increased risk of renal impairment and loss of antihypertensive efficacy
- Dual inhibition of the renin-angiotensin system: Increased risk of renal impairment, hypotension and hyperkalemia
- Potassium supplements/potassium-sparing diuretics: hyperkalemia
- Do not exceed doses greater than 20 mg daily of simvastatin
- mTOR inhibitors: increased risk of angioedema.
- Neprilysin inhibitors: increased risk of angioedema.Alcohol, barbiturates, or narcotics: Potentiation of orthostatic hypotension may occur.
- Cholestyramine and colestipol resins: Cholestyramine and colestipol resins bind the hydrochlorothiazide and reduce its absorption
- Corticosteroid, ACTH: Possible electrolyte depletion, particularly hypokalemia
- Pressor amines (e.g., norepinephrine): Possible decreased response to pressor amines.
- Skeletal muscle relaxants, nondepolarizing (e.g., tubocurarine): Possible increased responsiveness to the muscle relaxant
- Lithium: Generally should not be given with diuretics.

When used in pregnancy during the second and third trimesters, ACE inhibitors can cause injury and even death to the developing fetus. When pregnancy is detected, Zestoretic should be discontinued as soon as possible.
