What is XULTOPHY?
XULTOPHY is the brand name for insulin degludec and liraglutide and is available as a subcutaneous injection.
What dosages is XULTOPHY available in?
XULTOPHY is available as a single-patient-use pen that contains 100 units/mL of insulin degludec and 3.6 mg/mL of
liraglutide.
What is XULTOPHY used for?
XULTOPHY is an insulin / GLP-1 agonist and is used to treat type 2 diabetes.
How does XULTOPHY work?
XULTOPHY is a combination medication made up of two medications that lower blood sugar. Insulin degludec is an insulin. Insulin is a natural hormone your body normally makes to lower glucose (sugar) levels in the blood. It helps your body absorb and store sugar from your bloodstream after eating a meal. It also stops your liver from making glucose. Liraglutide is a GLP-1 agonist. It activates GLP-1, which tells your body to release more insulin, stop releasing sugar, and slow down digestion.
How do I take XULTOPHY?
The starting dose of XULTOPHY will depend on whether you’re already using a form of long-acting insulin or another GLP-1 agonist medication.
If you’re not taking insulin or a GLP-1 agonist: The usual starting dose is 10 units (10 units of insulin degludec/0.36 mg liraglutide) injected under the skin once a day.
If you’re taking insulin or a GLP-1 agonist: The usual starting dose is 16 units (16 units of insulin degludec/0.58 mg liraglutide) injected under the skin once a day.
Your provider will adjust your dose based on your blood sugar control and side effects. The maximum daily dose is 50 units per day (50 units of insulin degludec/1.8 mg of liraglutide).
Is it safe for me to take XULTOPHY?
XULTOPHY is a safe and effective treatment when used for FDA licensed indications. However, like all medications, they may give you unwanted side effects. You should always discuss potential side effects with your physician to ensure the medication is suitable and right for you. You should not use this medication if you have any of the following:
History or family history of thyroid cancer.
Rare disorder of the endocrine system called multiple endocrine neoplasia syndrome type 2 (MEN 2).
Symptoms of low blood sugar (hypoglycemia).
XULTOPHY Common Side Effects
Common side effects of XULTOPHY:
- Common cold or flu
- Headache
- Nausea
- Diarrhea
- Rise in pancreas enzyme
XULTOPHY Serious Side Effects
Serious side effects are rare with XULTOPHY. Contact your healthcare provider immediately if you experience any of the following.
Thyroid cancer: mass in the neck, trouble breathing or swallowing, hoarseness
Inflammation of the pancreas (pancreatitis): severe stomach pain that spreads to your back, nausea, vomiting
Low blood sugar: shaking, sweating, dizziness, confusion, blurred vision
Kidney damage: tiredness, dry and itchy skin, urinating more often, bloody or foamy urine, swelling in your feet or ankles, muscle cramping
Low potassium levels: weakness, muscle cramps or twitches, digestive problems, abnormal heartbeats, or kidney problems
Gallbladder problems: severe upper stomach and back pain, yellowing of your skin or eyes (jaundice), fever, nausea, vomiting
Heart failure: shortness of breath, tiredness, swelling in your legs, ankles, or feet, sudden weight gain
Serious allergic reactions: trouble breathing, hives, swelling of your face, lips, tongue, or throat
Effects of other drugs, drug classes and over-the-counter products on XULTOPHY
The following medications may interact with XULTOPHY:
- Drugs that affect glucose metabolism: Adjustment of XULTOPHY
- 100/3.6 dosage may be needed; closely monitor blood glucose.
- Anti-Adrenergic drugs (e.g., beta-blockers, clonidine
- guanethidine, and reserpine): Hypoglycemia signs and symptoms
- may be reduced or absent.
- Effects of delayed gastric emptying on oral medications:
- May impact absorption of concomitantly administered oral
- medications.

Is XULTOPHY safe in pregnancy?
It is not known if XULTOPHY can harm your unborn baby. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
