Gemfibrozil is a prescription medication in both the US and the UK. It requires a prescription (Rx) from a healthcare provider.

FDA approval of gemfibrozil

Gemfibrozil has been approved by the U.S. Food and Drug Administration (FDA) for the treatment of certain types of high cholesterol, including hypertriglyceridemia.

Is gemfibrozil available in the UK on NHS?

Gemfibrozil is used in the UK for the treatment of high cholesterol. It is available from the NHS with a prescription.

Mechanism of action

Gemfibrozil belongs to a class of medications known as fibrates. It works by reducing the production of triglycerides in the liver and increasing the levels of high-density lipoprotein (HDL) cholesterol, often referred to as good cholesterol.

Uses of gemfibrozil

Gemfibrozil is a medication that is primarily used to treat high levels of triglycerides in the blood. Triglycerides are a type of fat found in the blood, high levels can increase the risk of heart disease and pancreatitis. Gemfibrozil works by reducing the production of triglycerides in the liver and increasing the breakdown of triglycerides in the blood. It is often prescribed alongside lifestyle changes, such as a healthy diet and regular exercise, to help lower triglyceride levels. Additionally, gemfibrozil may also be used to increase levels of HDL cholesterol, commonly referred to as “good” cholesterol in individuals with low levels. However, it is important to note that gemfibrozil is not effective in treating low-density lipoprotein (LDL) cholesterol, which is often referred to as “bad” cholesterol. Overall, gemfibrozil is primarily used to manage high levels of triglycerides in the blood and improve overall cardiovascular health.

Dosage of gemfibrozil

The dosage of gemfibrozil will vary depending on the individual condition, response to the medication, and other factors. It is important to follow the dosage instructions provided by your healthcare provider. The medication is typically taken orally in tablet form and should be taken 30 minutes before your morning and evening meals. If you miss a dose, take gemfibrozil as soon as you remember. If it is near the time of the next dose, skip the missed dose. Take your next dose at the regular time. Do not double the dose to catch up.  Store at room temperature away from moisture, heat, and light.

Forms of gemfibrozil

Gemfibrozil is available in oral tablet form.

  • It is important to follow your healthcare professional instructions regarding the use of gemfibrozil and to undergo regular monitoring of liver function tests

Side effects of gemfibrozil

Common side effects of gemfibrozil may include stomach upset, abdominal pain, diarrhea, muscle pain, and increased liver enzyme levels. Reporting any persistent or serious side effects to your healthcare provider is important. Here are some of the common side effects associated with gemfibrozil:

  • The most common side effects of gemfibrozil is an upset stomach, which may cause symptoms such as nausea, vomiting, diarrhea, or abdominal pain. These symptoms are usually mild and temporary, but if they persist or worsen, it is important to consult a healthcare professional
  • Gemfibrozil has been known to cause muscle-related side effects, such as muscle pain, tenderness, or weakness. In rare cases, it can lead to a severe condition called rhabdomyolysis, which can cause muscle breakdown and kidney damage. If you experience any unexplained muscle pain or weakness, it is crucial to seek medical attention immediately
  • Gemfibrozil may affect liver function, leading to abnormal liver enzyme levels. Regular liver function tests are usually recommended for individuals taking this medication. If you experience symptoms like yellowing of the skin or eyes, dark urine, or persistent fatigue, contact your healthcare provider promptly
  • In rare cases, gemfibrozil can cause allergic reactions. Symptoms may include rash, itching, swelling, severe dizziness, or trouble breathing. If you experience any of these symptoms, seek medical attention immediately
  • Gemfibrozil may increase the risk of developing gallstones in some individuals. Symptoms may include abdominal pain, nausea, or vomiting. If you experience these symptoms, it is important to consult a healthcare professional

Drug interactions with other drugs

Here are some common medications that interact with gemfibrozil:

  • Gemfibrozil can increase the levels of statin (such as simvastatin, atorvastatin, pravastatin, rosuvastatin, lovastatin and fluvastatin) in the blood, leading to an increased risk of muscle-related side effects such as muscle pain, weakness, and breakdown. It is generally recommended to avoid using gemfibrozil with statins unless the potential benefits outweigh the risks and alternative therapies are not suitable
  • Gemfibrozil can enhance the effect of warfarin, an anticoagulant (blood thinner), leading to an increased risk of bleeding. Close monitoring of blood clotting time is necessary when gemfibrozil and warfarin are used concurrently, and adjustment of warfarin dosage may be required
  • Gemfibrozil can increase the concentration of certain oral antidiabetic agents, such as repaglinide, leading to an increased risk of hypoglycemia (low blood sugar). Dose adjustments and close monitoring of blood sugar levels are necessary when gemfibrozil is used with these medications
  • Gemfibrozil can increase the concentration of cyclosporine, an immunosuppressant used after organ transplantation. Close monitoring of cyclosporine levels and potential adjustment of cyclosporine dosage are necessary when gemfibrozil and cyclosporine are used together
  • Gemfibrozil can increase the levels of colchicine, a medication used to treat gout, leading to an increased risk of colchicine toxicity. It is generally recommended to avoid using gemfibrozil with colchicine unless the potential benefits outweigh the risks and alternative therapies are not suitable
  • Gemfibrozil can increase the levels of ezetimibe, a medication used to lower cholesterol absorption, leading to an increased risk of side effects. Close monitoring for ezetimibe-related side effects is necessary when gemfibrozil and ezetimibe are used together

It is important to inform your healthcare provider or pharmacist about all prescription drugs including over-the-counter meds and supplements you are taking to avoid any potential interactions.
