What is Tadalafil?
Tadalafil is a PDE5 inhibitor used to treat erectile dysfunction (ED). It is the active ingredient in Cialis. Tadalafil is unique among PDE5 inhibitors because of its long duration of action — up to 36 hours — earning it the nickname ‘the weekend pill’. It is available as an on-demand treatment (10 mg or 20 mg taken before sex) or as a daily low-dose treatment (2.5 mg or 5 mg taken every day). Tadalafil is also licensed for benign prostatic hyperplasia (BPH) and pulmonary arterial hypertension.
How does Tadalafil work?
Like sildenafil, tadalafil inhibits PDE5, increasing cGMP levels in the penile smooth muscle. This enhances blood flow to the penis during sexual stimulation. Tadalafil’s molecular structure gives it a much longer half-life (17.5 hours) compared to sildenafil (3–5 hours), which is why its effects last up to 36 hours. Sexual stimulation is still required — tadalafil does not cause spontaneous erections.
What is the difference between Tadalafil and Sildenafil?
Both are effective PDE5 inhibitors for ED. The main differences are: duration — tadalafil lasts up to 36 hours vs 4–6 hours for sildenafil; daily dosing option — tadalafil 5 mg daily provides continuous readiness; food interaction — tadalafil is not affected by food, while sildenafil’s absorption is delayed by fatty meals; side effects — tadalafil is more likely to cause back pain and muscle aches, while sildenafil causes more visual disturbances.
How do I take Tadalafil on demand?
Take one 10 mg or 20 mg tablet at least 30 minutes before sexual activity. The effect can last up to 36 hours. Do not take more than one dose per day. Tadalafil can be taken with or without food. The starting dose is usually 10 mg, increased to 20 mg if needed.
How does daily Tadalafil work?
Take one 2.5 mg or 5 mg tablet at the same time every day, regardless of sexual activity. After about 5 days, tadalafil reaches a steady state in the blood, providing continuous readiness for sexual activity without timing doses around sex. Daily dosing is preferred by men who have sex frequently (more than twice a week) and by those who also have BPH symptoms.
What are the side effects of Tadalafil?
Common side effects include headache, facial flushing, nasal congestion, indigestion, back pain, and muscle aches (particularly in the legs). Back pain and myalgia are more common with tadalafil than with other PDE5 inhibitors and typically occur 12–24 hours after taking the dose. These side effects usually resolve within 48 hours. Daily low-dose tadalafil tends to cause fewer side effects.
Who should not take Tadalafil?
Tadalafil must NOT be taken with nitrate medications or recreational nitrates (poppers). It should be used cautiously in men with severe cardiovascular disease, recent stroke or MI, severely low blood pressure, or liver/kidney impairment. Alpha-blockers for BPH require careful dose titration with tadalafil. Always disclose all medications to your prescriber.
