FAQs

Answers to popular questions about Sildenafil

What is Sildenafil?

Sildenafil is a PDE5 inhibitor used to treat erectile dysfunction (ED) in men. It is the active ingredient in Viagra and is now widely available as a generic medication. Sildenafil works by increasing blood flow to the penis during sexual stimulation. It is available as 25 mg, 50 mg, and 100 mg tablets. Sildenafil 50 mg is available over the counter from pharmacies as Viagra Connect, while other strengths require a prescription.

How does Sildenafil work?

Sildenafil inhibits phosphodiesterase type 5 (PDE5), an enzyme that breaks down cyclic GMP (cGMP) in the smooth muscle of the penis. During sexual arousal, nitric oxide is released, which activates cGMP to relax smooth muscle and increase blood flow to the penis, causing an erection. By blocking PDE5, sildenafil maintains higher cGMP levels, making it easier to achieve and maintain an erection. It does not cause an automatic erection — sexual stimulation is still required.

How effective is Sildenafil?

Sildenafil is effective in approximately 70–80% of men with erectile dysfunction. It works across a wide range of ED causes, including vascular, neurological, psychological, and diabetes-related ED. If the 50 mg dose is insufficient, your prescriber may increase it to 100 mg. If 100 mg does not work after several attempts (at least 4–8 attempts are recommended), alternative PDE5 inhibitors or treatments may be considered.

How do I take Sildenafil?

Take one tablet approximately 30–60 minutes before sexual activity. The effect lasts for about 4–6 hours. Do not take more than one dose in 24 hours. Sildenafil can be taken with or without food, but high-fat meals may delay its absorption and onset of action. Alcohol should be limited as excessive consumption can impair erectile function and worsen side effects.

What is the usual starting dose of Sildenafil?

The usual starting dose is 50 mg. Based on effectiveness and tolerability, the dose may be increased to 100 mg or decreased to 25 mg. Men over 65, those with liver or kidney impairment, or those taking certain medications (e.g., alpha-blockers) should start at 25 mg. Your prescriber will advise the most appropriate dose for you.

What are the side effects of Sildenafil?

Common side effects include headache, facial flushing, nasal congestion, indigestion, and dizziness. Visual disturbances (blue-tinted vision) may occur, particularly at the 100 mg dose. These side effects are usually mild and temporary. Rare but serious side effects include priapism (prolonged erection lasting more than 4 hours — seek emergency medical help), sudden hearing loss, and non-arteritic anterior ischaemic optic neuropathy (NAION).

Who should not take Sildenafil?

Sildenafil must NOT be taken with nitrate medications (e.g., GTN spray, isosorbide mononitrate) or recreational nitrates (poppers/amyl nitrite), as this can cause a dangerous, life-threatening drop in blood pressure. It should be used with caution in men with severe cardiovascular disease, recent stroke or heart attack (within 6 months), severely low blood pressure, or retinitis pigmentosa. Always disclose your full medical history to your prescriber.

Can I buy Sildenafil without a prescription?

Sildenafil 50 mg is available over the counter as Viagra Connect from pharmacies after a pharmacist consultation. Other strengths (25 mg and 100 mg) and generic sildenafil require a prescription. If you need dose adjustment, an online service like NowPatient can provide a prescription for the appropriate strength with convenient home delivery.

What is the difference between Sildenafil and Viagra?

Sildenafil is the active ingredient — Viagra is the original brand name manufactured by Pfizer. Since the patent expired, generic sildenafil has become widely available at a significantly lower cost. Generic sildenafil is bioequivalent to Viagra and has the same effectiveness and safety profile. Most prescriptions are now for generic sildenafil.

How can I get Sildenafil?

Sildenafil can be obtained over the counter as Viagra Connect 50 mg, or on prescription from your GP, sexual health clinic, or through NowPatient. NowPatient offers discreet online consultations and home delivery of prescription-strength sildenafil in all available doses.
