FAQs
Answers to popular questions about Tolterodine
What is Tolterodine?
Tolterodine is an antimuscarinic medication used for the treatment of overactive bladder (OAB) with symptoms of urgency, frequency, and urge incontinence. It is available as immediate-release tablets (1 mg, 2 mg) and modified-release capsules (4 mg). Brand names include Detrusitol and Detrusitol XL. Tolterodine is one of the most commonly prescribed antimuscarinics for OAB in the UK and is relatively bladder-selective.
How do I take Tolterodine?
Immediate-release: 2 mg twice daily, reduced to 1 mg twice daily if side effects are problematic. Modified-release: 4 mg once daily. Swallow with water, with or without food. The modified-release formulation is preferred as once-daily dosing is more convenient and generally better tolerated. Effects on urgency and frequency are usually noticed within 1–2 weeks, with full benefit at 4–8 weeks.
What are the side effects of Tolterodine?
Common side effects include dry mouth (most frequent), constipation, headache, abdominal pain, dyspepsia, dry eyes, and dizziness. Tolterodine causes less dry mouth than oxybutynin. In elderly patients, antimuscarinics may increase the risk of cognitive impairment, confusion, and falls — this is an important consideration. If dry mouth or constipation are troublesome, consider switching to mirabegron (Betmiga), which has a different mechanism and avoids these anticholinergic effects.
Who should not take Tolterodine?
Tolterodine is contraindicated in patients with urinary retention, uncontrolled narrow-angle glaucoma, myasthenia gravis, severe ulcerative colitis, and toxic megacolon. Caution is needed in patients with significant bladder outflow obstruction, gastrointestinal obstructive disorders, severe hepatic impairment, renal impairment, and those at risk of QT prolongation.
How can I get Tolterodine?
Tolterodine requires a prescription from your GP, urologist, or online service like NowPatient. It is available on the NHS. Generic tolterodine is widely available and less expensive than the branded Detrusitol. Your prescriber will assess your OAB symptoms and previous treatments before prescribing.
