FAQs

Answers to popular questions about Duloxetine

What is Duloxetine?

Duloxetine is a serotonin and noradrenaline reuptake inhibitor (SNRI) available as gastro-resistant capsules in strengths of 20 mg, 30 mg, 40 mg, and 60 mg. It is licensed in the UK for the treatment of major depressive disorder, generalised anxiety disorder, diabetic peripheral neuropathic pain, and stress urinary incontinence (as branded Yentreve 40 mg). Duloxetine is a prescription-only medicine. Brand names include Cymbalta and Yentreve.

How does Duloxetine work?

Duloxetine inhibits the reuptake of both serotonin and noradrenaline in the brain and spinal cord, increasing the availability of these neurotransmitters. For depression and anxiety, this enhances mood regulation and reduces excessive worry. For neuropathic pain, the increased noradrenaline and serotonin activity in descending pain inhibitory pathways reduces pain signal transmission. For stress urinary incontinence, duloxetine increases the tone of the urethral sphincter through its action on Onuf’s nucleus in the sacral spinal cord.

What is Duloxetine used for?

Duloxetine is licensed for major depressive disorder (60 mg daily), generalised anxiety disorder (30–60 mg daily), diabetic peripheral neuropathic pain (60 mg daily), and stress urinary incontinence (as Yentreve 40 mg twice daily). Off-label uses include chronic pain conditions such as fibromyalgia and chronic musculoskeletal pain. The dose depends on the indication and should be determined by your prescriber.

How do I take Duloxetine?

Swallow duloxetine capsules whole with water — do not crush, chew, or open them, as the gastro-resistant coating prevents degradation in stomach acid. Take at the same time each day, with or without food. For depression: the usual dose is 60 mg once daily. For anxiety: start at 30 mg daily, increasing to 60 mg after 2 weeks if needed. For neuropathic pain: 60 mg once daily (start at 30 mg for 2 weeks if preferred). For stress urinary incontinence: 40 mg twice daily (as Yentreve).

How long does Duloxetine take to work?

For depression and anxiety, duloxetine may begin to improve symptoms within 1–2 weeks, but full therapeutic effect usually takes 4–6 weeks. For neuropathic pain, some improvement may be noticed within the first week, with maximum benefit at 4–8 weeks. For stress urinary incontinence, benefits are typically seen within 2–4 weeks. Continue taking duloxetine as prescribed even if you do not feel immediate improvement.

What are the common side effects of Duloxetine?

Very common side effects include nausea (most common, especially in the first 1–2 weeks), headache, dry mouth, and somnolence (drowsiness). Common side effects include dizziness, constipation, diarrhoea, fatigue, decreased appetite, insomnia, increased sweating, and sexual dysfunction (reduced libido, difficulty reaching orgasm, erectile dysfunction). Nausea usually improves within the first 1–2 weeks. Taking duloxetine with food may help reduce nausea.

Can I stop Duloxetine suddenly?

No, duloxetine should not be stopped abruptly. Sudden discontinuation can cause withdrawal symptoms including dizziness, nausea, headache, paraesthesia (tingling or electric shock sensations, sometimes called ‘brain zaps’), irritability, insomnia, anxiety, and diarrhoea. The dose should be gradually tapered over at least 2 weeks under your prescriber’s guidance. A common tapering schedule is reducing to the next lower dose for 2 weeks before stopping completely.

Who should not take Duloxetine?

Duloxetine should not be taken by people currently taking or recently stopped taking MAO inhibitors (allow 14 days washout), those with uncontrolled narrow-angle glaucoma, severe hepatic impairment, severe renal impairment (creatinine clearance <30 mL/min), or those taking fluvoxamine (strong CYP1A2 inhibitor). Caution is needed in patients with hypertension (duloxetine can increase blood pressure), bleeding disorders, epilepsy, and bipolar disorder. It should be used with caution in pregnancy.

Can I drink alcohol with Duloxetine?

Alcohol should ideally be avoided while taking duloxetine. Both substances affect the central nervous system, and combining them may increase drowsiness, dizziness, and impaired judgement. Alcohol can also worsen depression and anxiety. Additionally, both duloxetine and alcohol are metabolised by the liver, and the combination may increase the risk of liver toxicity. If you choose to drink, keep consumption minimal.

How can I get Duloxetine?

You can request Duloxetine through NowPatient by completing a short online consultation with one of our prescribing clinicians. Your clinician will review your condition, your mental health history and any other medicines you take to decide whether Duloxetine is suitable for you. If treatment is suitable, a prescription is issued and your medication is dispensed by our registered pharmacy and delivered to your door.
