Data Agreement
If you are registered with a GP practice then you will have a summary care record unless you chose not to. Your SCR is a read only electronic record that holds important clinical information, created by your GP.
This can only be accessed by authorised staff with your permission. If you would like to view or add information that you feel will help your health care then please visit your GP practice. Having access to your records improves care, is safer and reduces the risk of prescribing errors.
Your SCR contains the following basic information:
- Medication you are taking
- Any allergies you may have
- Any adverse reactions you may have to medication
Certain medical conditions can get free NHS prescriptions with A valid medical exemption certificate.
A medical exemption certificate:
- Entitles you to free NHS prescriptions only
- Doesn't cover dental treatment or help with other health costs
- Should be shown when you collect a prescription
- Is valid for five years ( or until your 60th birthday, whichever is sooner )
It is your responsibility to check the expiry date, if you claim free prescriptions after your certificate expires, you could have to pay a penalty charge of up to £100.
